2010-2011年医院抗菌药物使用调查分析

摘　　要：目的调查医院2010-2011年临床科室抗菌药物使用和细菌学检查情况,分析存在的问题和不足,指导医院制定有效的调查控制措施,以安全、合理地使用抗菌药物。方法随机抽取2010和2011年门诊处方2753张及出院病历1225份,对抗菌药物使用和细菌学检查进行回顾性调查分析。结果 2010-2011年抗菌药物使用率分别为门诊1519例占27.7%、住院1639例占66.9%;非手术1399例占57.1%、手术2129例占86.9%、Ⅰ类切口手术1520例占71.0%,其中术前30min～2h内使用984例占66.7%;抗菌药物使用强度为61DDD;细菌学送检579例,送检率为35.3%。结论医院细菌学送检率已达标,但抗菌药物使用率过高,尤其是手术科室更明显,虽然2011年低于2010年,仍要采取切实有效的措施,特别是加强外科等手术科室管理,搞好抗菌药物的合理使用。OBJECTIVE To investigate the clinical use of antibiotics and etiology of inspection in the hospital from 2010 to 2011 so as to provide evidence for the standard management and the rational use of antibiotics. METHODS By using the retrospective methodology, totally 2753 copies of outpatient service prescriptions and 1225 medical records of the discharged cases were randomly selected from 2010 to 2011, then the use of antibiotics and the bacteriological examination were retrospectively investigated. RESULTS From 2010 to 2011, the utilization rate o{ antibiotics was 27. 7% （1519 cases） in the outpatient department and 66. 9% （1639 cases） of the hospitalized patients ; there were 1399 （57.1%） cases of patients who did not receive surgery, 2129 （86.9 %） cases of patients who underwent surgery, 1520 （71.0%） cases of patients who underwent the type I incision surgery, and 984 （66. 7%） cases of patients who used antibiotics 30rain-2 hours be{ore the surgery. The intensity of use of antibiotics was 61 DDD; there were 579 （35. 3%） cases of patients who were submitted for the bacteriology. CONCLUSION The submission rate of the bacteriology has met the standard, but the utilization rate of antibiotics is excessively high, especially in the operating room, although it is lower in 2011 than in 2010, we should take effective measures, especially strengthening the management of operating room, so as to reasonably use the antibiotics.
