摘  要:目的:研究外阴白色病变组织中血管内皮生长因子(VEGF)和人原始造血细胞抗原(CD34)的表达,从而进一步探讨外阴白色病变的发病机制。方法:应用免疫组化方法观察62例外阴白色病变(包括外阴鳞状上皮增生32例,外阴硬化性苔藓30例)组织中的VEGF、CD34表达情况,10例正常外阴皮肤组织作为对照。结果:外阴硬化性苔藓(LS组)的VEGF的表达和微血管数目(MVD值)均显著低于正常外阴皮肤组及外阴鳞状上皮增生(SH组),差异有统计学意义(P<0.05);SH组与正常外阴组织相比差异无统计学意义(P>0.05),VEGF的表达与MVD计数在LS中存在正相关。结论:外阴硬化性苔藓组织中VEGF表达减少,引起MVD值显著减少,可能是外阴硬化性苔藓的发病机制之一;外阴鳞状上皮增生组织中VEGF的表达和MVD值无明显改变,微血管结构功能障碍可能是外阴鳞状上皮增生的发病机制之一。Objective: To research the expression of VEGF and CD34 in vulvar white lesions and to discuss the pathogenesis of the vulvar white lesions. Methods : Immunohistochemical method was used to obse^e the expressions of VEGF and CD34 in 62 patients with vul- var white lesions (including 32 cases of vulval squamous epithelial hyperplasia and 30 cases of vulval lichen sclerosis) , 10 cases with nor- mal vulval tissue were selected as control group. Results: The expression of VEGF and microvessel density (MVD) of lichen sclerosis of vulva was statistically significantly lower than that in normal vulva group ( P 〈 0. 05 ) and that in vulval squamous epithelial hyperplasia group, the difference was statistically significant ( P 〈 0. 05) . There were no statistically significant difference in the expression of VEGF and MVD between vulval squamous epithelial hyperplasia group and normal vulva group ( P 〉 0. 05 ) , VEGF expression and MVD count were positive correlated in LS. Conclusion : The expression of VEGF in vulva lichen sclerosis tissue has decreased, which lead to the decreasing of MVD, this may be one of the pathogenesis of vulva lichen sclerosis ; the expression of VEGF and MVD in vulval squamous epithelial hy- perplasia tissue has no obvious change, microvascuIar structure dysfunction may be one of the pathogenesis of squamous epithelial hyperpla- sia.
