几丁糖加球囊导尿管对预防重度宫腔粘连分离术后再粘连的效果

摘　　要：目的:探讨重度宫腔粘连宫腔镜分离术后应用几丁糖配合球囊导尿管预防宫腔再粘连的疗效。方法:回顾性分析2005年1月～2010年1月在湖北医药学院附属人民医院宫腔镜确诊的68例重度宫腔粘连患者行宫腔镜下粘连分离术患者的相关临床资料,根据术后预防再粘连方法的不同,分为A组(球囊导尿管配合几丁糖组)和B组(球囊导尿管组),比较两组术后疗效及术后1、3、6个月随访宫腔再次粘连情况。结果:两组疗效比较,A组的治愈率及有效率明显高于B组,术后随访宫腔镜下宫腔形态,A组发生再粘连的几率显著低于B组,差异有统计学意义(P<0.05)。结论:重度宫腔粘连分离术后持续放置球囊导尿管并加用几丁糖预防术后再次发生宫腔粘连效果更佳,优于单纯使用球囊组,是安全、有效的方法,值得临床推广。Objective ： To explore the effects of combination of chitosan and sacculs urinary catheter after trans - cervical resection of adhesions （TCRS） to treat the severe intrauterine adhesion （IUA） . Methods： 68 cases of severe intrauterine adhesions with hystero- scopic diagnosis and underwent hysteroscopic adhesions dissection in our hospital from January 2005＇ - January 2010 were collected , and their clinical data were analyzed retrospectively. According to the prevention of adhesions after different methods, all cases were divided into Group A （ saccule urinary catheter with chitosan group） and group B （ saccule urinary catheter group）, and the recurrence of intrauterine adhesions were compared after treatment and when follow - up 1 month, 3 months and 6 months. Results： Comparison of the two groups, the cure rate and effective rate in Group A was significantly higher than that in Group B, hysteroscopic uterine morphology of patients were follow - up, the recurrence of adhesions in Group A was significantly lower than in that in Group B, the difference was statistically signifi- cant （ P 〈 0. 05 ） . Conclusion ： Combination of continuously placing saccule urinary catheter and chitosan after trans - cervical resection of adhesions to treat severe intrauterine adhesions is better than using the saccule urinary catheter group, it is a safe and effective method, it is worth clinical practice.
