健康成人碎裂QRS波的发生及影响因素分析

摘　　要：目的：研究健康成人碎裂QRS波的发生率、分布规律及影响因素。方法：本研究入选1491名健康成年体检者，年龄20～88岁，其中男性878名，女性613名，采集静息状态下常规12导联心电图。观察碎裂QRS波发生率及性别差异。将入选者分为〈50岁者和≥50岁者两个年龄段，分析其碎裂QRS波的发生率、分布规律及影响因素。结果：碎裂QRS波发生率：①1494名发生率9．66％（144／1491），男、女性发生率分别为11．50%（101／878）和7．01％（43／613）；②≥50岁者碎裂QRS波发生率明显高于〈50岁者（13．29％vs7．88％），女性发生率增加尤为明显（11．73％vs4．50％），男性不明显（13．33％VS10．55％）；③碎裂QRS波发生在下壁导联的比例为90．97％（131／144），发生在胸前导联的比例为8．33％（12／144），均多在V1～V3导联。高侧壁导联发生率7．64％（11／144），几乎均伴随下壁导联发生。结论：①碎裂QRS波在健康成人中比较常见，并且多发生在下壁导联；②碎裂QRS波发生与年龄和性别相关。Objective： To analyze the prevalence and its influencing factors for fragmented QRS （FQRS） complex in heahhy aduhs. Methods： A total of 1491 healthy subjects from physical examination were prospectively studied for their FQRS by routine 12-1cad ECG at the rest condition. The subjects were at the age of （20-88） years including 878 male and 613 female. The subjects were divided into 2 groups as age〈50 years and age≥50 years. The FQRS occurrence, distribution and the influencing factors were studied in all subjects. Results： ① The overall FQRS prevalence was 9.66% （144/1491）, the occurrence rate in male was 11.50% （101/878） and in female was 7.01% （43/613）. ② FQRS occurrence rate was higher in subjects≥50 years group than that in subjects〈50 years group, 13.29% vs. 7.88%, and the female was higher than male, 11.73% vs. 4.50%. ③ There 90.97% of FQRS occurred in inferior leads, 8.33% （12/144） in precordial leads, more in V1-V3 lead. There were 7.64 % （l 1/144） in high side wall leads, and almost with the inferior leads. Conclusion： ① FQRS was common in healthy adults and it more occurred in inferior leads. ② FQRS prevalence was related to age and gender.
