厄贝沙坦氢氯噻嗪对血管内皮功能和高敏C反应蛋白的影响

摘　　要：目的观察厄贝沙坦氢氯噻嗪对血管内皮功能和高敏C反应蛋白(hs-CRP)的影响。方法选择1级、2级原发性高血压患者100例,分为氢氯噻嗪组46例及厄贝沙坦氢氯噻嗪组54例,2组患者每日晨起分别口服氢氯噻嗪25mg及厄贝沙坦氢氯噻嗪1片(每片含厄贝沙坦150mg,氢氯噻嗪12.5mg),连续6个月,检测2组患者治疗前后血压、血生化、hs-CRP、血管性血友病因子(vWF)等指标,以及肱动脉内皮依赖性舒张功能(EDD)的变化。结果与治疗前比较,氢氯噻嗪组患者治疗后收缩压、舒张压、血钾明显减低,尿酸明显升高(P<0.01),vWF、EDD差异无统计学意义(P>0.05);厄贝沙坦氢氯噻嗪组治疗后收缩压、舒张压、hs-CRP、vWF明显减低(P<0.05,P<0.01),EDD明显提高(P<0.01)。结论厄贝沙坦氢氯噻嗪降压的同时可以改善受损的血管内皮功能、减少炎性因子。Objective To observe the effect of irbesartan hydrochlorothiazide tablets on endothelial function and hs-CRP in hypertensive patients. Methods One hundred patients with mild-moder- ate essential hypertension were divided into hydrochlorothiazide treatment group （n = 46） and irbesartan hydrochlorothiazide treatment group （n = 54）. Patients in hydrochlorothiazide treat- ment group administered an oral hydrochlorothiazide tablet （25 mg） and those in irbesartan hydrochlorothiazide treatment group took an oral irbesartan hydrochlorothiazide tablet in the morning for 6 months. Their blood pressure,serum hs-CRP and yon Willebrand factor （vWF） lev- els,brachial artery endothelium-dependent dilation （EDD）, and blood biochemical criteria were measured before and after treatment. Results The systolic and diastolic blood pressure and serum K level were significantly lower whereas the uric acid level was significantly higher in hydrochlo- rothiazide treatment group after treatment than before treatment （P〈0.01） with no significant difference in vWF level and EDD （P〈0.05）. The systolic and diastolic blood pressure and serum hs-CRP and vWF level were significantly lower whereas the EDD was significantly higher in irbe- sartan hydrochlorothiazide treatment group after treatment than before treatment （P〈0.05,P〈 0.01）. Conclusion Irbesartan hydrochlorothiazide tablets can decrease the blood pressure, im- prove the injured vascular endothelial function, and reduce the inflammatory factors in hyperten- sive patients.
