摘　　要：目的探讨失效模式和效应分析(FMEA)在降低手术审批流程管理中的缺陷的应用效果。方法使用FMEA管理方法 ,对外科手术医生的手术审批缺漏的失效模式进行分析、评估,针对影响手术审批的各项高危因素,修订医院医疗制度、再造审批流程、并结合信息系统实现对审批的重点环节的有效监督,半年后比较评价应用前后RPN值及手术审批效果。结果经过改进的半年内,未发生重大疑难手术未审批或审批不当,手术审批的总RPN值下降了90%。结论应用FMEA模式能够有效防控手术审批中的不规范现象,改善医疗质量。Objective： To investigate the effect of failure mode and effect analysis （FMEA） application in reducing defects in the management of operation approval process. Methods： The failure mode of the defects of the operation approval of operation doctors was analyzed and evaluated using the FMEA management method. In view of the high risk factors affecting operation approval, the related regulations of the hospital were revised and the approval process reengineered. With the help of the information system, the important links for approval were supervised and the RPN values before and after the application were compared in the half a year application period. Results： During the application period, no major, difficult operation was done without approval or improper approval and the total RPN decreased by 90%. Conclusion： Application of FMEA mode can effectively control the defects in operation approval and improve care quality.
