新型人感染H7N9禽流感中医证候分布规律及辨证论治思路

摘　　要：目的:分析新型人感染H7N9禽流感中医证候分布规律,探讨辨证论治思路。方法:收集新型人感染H7N9禽流感18例患者中医症状、舌象、脉象等资料,进行描述性分析。结果:新型人感染H7N9疾病早期,主要以肺热壅盛证(33.3%)、燥热伤肺证(33.3%)为主要证型,并可见热灼营阴证(11.1%)、热陷心包证(11.1%)等营分证候;中期肺卫证候消失,肺热壅盛(23.5%)仍见,燥热伤肺证(41.2%)为主,可出现肺热腑实的阳明热证(11.8%);后期多见气阴两伤证(61.5%)。结论:新型人感染H7N9禽流感证候分布具有一定规律,辨证论治应把握卫气营血传变特点。Objective： This study aimed to analyze the TCM syndrome features distribution of novel H7N9 avian influenza virus infection and discuss the syndrome differentiation and treatment thoughts in this special population. Methods： 18 H7N9-infected patients were included in this observational study. The symptom, tongue presentations and pulse presentations are descriptively studied. Results： The main syndromes of H7N9-infected patients on the onset of disease are syndrome of exuberance of lung heat （33.3%） and dry-heat impairing lung （33.3%）, concomitant with heat burning ying yin syndrome （11.1%） and syndrome of invasion of pericardium by heat （11.1%）. The proportion of syndrome of exuberance of lung heat （23.5%） and syndrome of dry-heat impairing lung （41.2%） is the main syndrome in the mid-stage of H7N9 infection. Syndrome of accumulation of excessive heat in the lung and fu-organs （11.8%） appears in this period. The main syndrome of advanced stage in H7N9-infected patients is deficiency of qi and yin syndrome （61.5%）. Conclusion： The distribution of syndromes of novel H7N9 avian influenza virus infection is regularly. Syndrome differentiation and treatment thoughts should base on the transformation and development specialties of wei-qi-ying-blood.
