摘  要:目的:观察电针对坐骨神经损伤大鼠脊髓腹角中生长相关蛋白43(GAP-43)的影响,探讨电针治疗坐骨神经损伤的生物学机制。方法:采用大鼠坐骨神经夹持损伤模型,通过斜板试验观察大鼠的行为学变化;通过免疫组织化学染色观察大鼠脊髓腹角中GAP-43表达情况,进而统计分析各组间的差异。结果:造模后7 d模型组大鼠斜板试验结果表明,坐骨神经损伤后大鼠的运动功能明显降低(P<0.01);电针治疗10次后,电针治疗组与模型组和模型对照组相比均有显著性升高(P<0.01),且已接近正常组水平;电针治疗20次后,电针治疗组已达到甚至超过正常组水平,且与模型组和模型对照组相比仍有显著性差异(P<0.01)。免疫组化结果:造模后7 d,模型组与正常组和假手术组相比,GAP-43表达有明显升高(P<0.05);电针治疗10次、20次后,电针治疗组的GAP-43表达与模型组比较有显著性差异(P<0.05),与模型对照组相比也有明显升高(P<0.05)。结论:电针可以通过提高相应节段脊髓腹角中GAP-43的释放,从而促进神经元的发育和再生,最终改善坐骨神经损伤大鼠的运动功能。Objective : Through the research on changes of GAP - 43 expression in ventral horn of spinal cord of sciatic nerve injury rats, the study can explore the biological mechanisms of electric acupmlcture on the treatment of sciatic nerve injury. Methods : After making the model of sciatic nerve clamping damage, we observed the rats behavior change through inclined plane test. Through the research on changes of GAP -43 expression in ventral horn of spinal cord, approach and mechanism were revealed by electric acupuncture on sciatic nerve injury. Results : Seven days after making the model, inclined plate test results showed that movement function of sciatic nerve injury rats was significantly declined( P 〈 0.01 ) Ten days after therapy, the results of therapy group were obviously higher than that of model group and model control group(P 〈 0.01 ), and already close to normal group. Twenty days after therapy, the results of therapy group had reached even more than normal group level, and compared with the model group and model control group there were still significant difference ( P 〈 0.01 ). The results of GAP - 43 showed that : after seven days of sciatic nerve injured, GAP - 43 expression of model group was significantly increased compared with the normal group and sham - operation group (P 〈 0. 05 ). After ten and twenty days of electric acupuncture therapy, the results of GAP -43 from electric acupuncture therapy group had significant difference compared with model group( P 〈 0. 05 ), and significantly increased than that in model control group ( P 〈 0. 05 ) . Conclusion : Electric acupuncture can promote the expression of GAP - 43 in ventral horn of spinal cord and the regeneration of injured nerves, consequently provide a good microenvironment for the iniured nerve, and in the end enhance the sensory function of rats with sciatic nerve injury.
