补肾解郁法治疗肾虚肝郁型抑郁症临床疗效观察

摘　　要：目的:探讨益肾补虚、疏肝解郁类中药组方治疗肾虚肝郁型抑郁症的疗效。方法:采用补肾解郁方(黄芪、西洋参、白术、天麻、枸杞子、熟地、淫羊藿、白芍、酸枣仁、茯苓、知母、香附、柴胡、郁金等)治疗肾虚肝郁型抑郁症240例,并与氟西汀对照,用药疗程均为12周。在治疗3、6、12周时,对两组的汉密顿(HAMD)减分疗效和中医证候疗效进行对比观察。结果:经过3周治疗,补肾解郁组有效率低于氟西汀组(P<0.05)。6周末,两组痊愈率、显效率、有效率、无效率均差异不显著(P>0.05)。12周末,补肾解郁组痊愈率、显效率、HAMD减分值和疗效指数(EI)显著高于氟西汀组(P<0.05)。不良反应明显低于氟西汀组(P<0.05)。结论:补肾解郁法治疗肾虚肝郁型抑郁症具有良好的临床疗效。Objective：To explore the clinical curative effect in depression patients with kideny deficiency and liver stagnation treated with traditional Chinese medicine formula for benefiting kidney and supplementing deficiency, soothing liver and resolving depression. Methods ： 240 depression patients with kideny deficiency and liver stagnation were treated with prescription for tonifying kidney and resolving depression （ experimental group, astragalus mongholicus, american gin- seng,white atractylodes rhizome, gastrodia elata, the fruit of Chinese wolfberry, Rehmannia Glutinosa, epimedium, radix paeoniae alba, spina date seed, Poria cocos, rhizoma anemarrhenae, rhizoma cyperi, radix bupleuri, radix curcumae, and so on）, and compared with the effect of Fluoxetine （control group）, and drug treatment was 12 weeks. The curative effect on Hamilton Depression Scale （HAMD） and traditional Chinese medicine syndrome were observed before and after treatment for 3 weeks, 6 weeks, 12 weeks. Results ： （ 1 ） Treatment for 3 weeks, the efficient rate on patients in the experimental group were significantly lower than those in the control group （ P 〈 0. 05 ）. The healed rate, significant efficiency rate, efficient and without efficiency rates in the two groups showed no significant differences at 6th weekend （ P 〉 0. 05 ）. Treatment for 12 weeks ,the healed ,significant efficiency rates ,decreased score of HAMD and effect index （El） on patients in the ex- perimental group were significantly higher than those in the control group （ P 〈 0. 05 ）. The untoward effect on patients in the experimental group was significantly lower than those in the control group （P 〈 0. 05）. Conclusion：There is a favorable clinical curative effect in depression patients with kideny deficiency and liver stagnation treated with the method of tonifying kidney and resolving depression
