冠心合剂联合PCI对冠心病患者的症状、安全性及IGF-1影响

摘　　要：目的:观察冠心合剂联合PCI治疗冠心病患者的疗效、安全性,并探讨冠心合剂治疗作用的可能机理。方法:研究对象为44例经冠状动脉造影确诊为冠心病并行冠脉介入治疗(PCI)治疗的患者,经中医辨证符合气虚血瘀的证型。随机分为单纯PCI组和PCI+冠心合剂组。患者入选后记录其主诉胸痛、胸闷、心悸的症状和舌质情况及实验室检查结果(ck,ck-MB,AST、sCr水平),采用酶联免疫吸附法(ELISA)测定血胰岛素样生长因子-1水平(IGF-1)。3个月后记录随访情况,包括患者主诉症状和舌质情况,比较患者实验室检查结果及IGF-1的变化。结果:PCI+冠心合剂组治疗前后患者症状改善情况具有显著统计学意义(P<0.001),且较单纯PCI组改善明显,具有统计学意义(P<0.05)。PCI+冠心合剂组患者IGF-I水平较干预前升高均具有显著统计学意义(P<0.001),较单纯PCI组升高明显,具有统计学意义(P<0.05)。患者在接受治疗期间患者ALT、AST、HS-CRP、LDL-C改变情况无明显差异。结论:PCI联合冠心合剂明显改善冠心病患者临床症状,具有较好的安全性,改善心肌缺血作用机理可能和升高IGF-I水平有关。Objective ： To observe the clinic effect and safety of Guanxin Decoction on the patients of coronary heart disease and analyse its possible mechanism. Methods ：44 patients with coronary heart disease diagnosed by coronary artery angiography were consistent with the Qi deficiency and blood stasis in traditional Chinese medicine. The degree of coronary artery stenosis was evaluated using coronary artery angiograpby. These patients were divided into 2 groups：25 patients were treated with the western medicine after percutaneous coronary intervention （PCI） ;19 patients were treated with PCI and Guanxin Decoction~ The results of lab assay and serum IGF - 1 of all patients were recorded. After 3 months follow - up, the complaints and the result of the lab assay were recorded. Results ： The symptoms of patients treated by PCI with Guanxin Decoction were relieved（P 〈0. 001 ）. In the PCI and Guanxin Decoction group,symptoms were improved better than PCI only（P 〈 0. 001 ）. Before the treatment, the IGF - 1 levels in each group had no difference. After PCI and Guanxin Decoction treatment, the serum IGF - 1 was increased（ P 〈 0. 001 ） , and the serum IGF - 1 raised than that in the PCI group（P 〈 0.05 ）. During the treatment, levels of ALT, AST, CK and CK - MB had no difference （ P 〉 0. 05 ） ~ Conclusion ： The PCI and Guanxin Decoction can get better clinical results for the patients of coronary heart disease safe- ly. Its mechanism may be related with IGF - 1.
