健脾益肾活血中药干预治疗糖尿病肾病大鼠的实验研究

摘　　要：目的探讨健脾益肾活血中药对2型糖尿病肾病大鼠肾脏保护作用的机理。方法采用链脲佐菌素+高脂饲料诱导糖尿病肾病(DN)大鼠动物模型,造模成功后分为4组:正常对照组、模型组、贝那普利组和中西药结合治疗组。贝那普利组给予贝那普利10 mg/kg灌胃,中药组加予中药煎剂,连续给药20周后观察大鼠一般状态,检测血清和尿液中BUN、Cr、TC、TG、24 h尿蛋白、HbAlc和FINS水平,并取肾组织染色后行病理检验,观察其组织学改变,测定肾组织内TGF-β1、Col-Ⅳ的表达。结果中西药结合治疗组实验大鼠一般状态较模型组改善较为明显,贝那普利组和中西药结合治疗组BUN、Cr、TC、TG、24 h尿蛋白、HbAlc和FINS均有所下降,其中中西药结合治疗组指标下降更明显(P<0.05,P<0.01),模型组肾脏形态学病理检测结果观察有明显病变,中西药结合治疗组和贝那普利组有不同程度改善,中西药结合治疗组的TGF-β1、Col-Ⅳ的表达下降明显,与模型组有极显著差异(P<0.01)。结论健脾益肾活血中药可改善2型糖尿病肾病大鼠肾脏损害,延缓肾功能损害进程,对DN大鼠肾功能具有保护作用。Objective To explore the mechanism of traditional Chinese medicine of invigorating spleen and tonifying kidney and activating blood on the renal protective effect on diabetic nephropathy in type 2 diabetes mellitus rats. Methods Using streptozotocin ＋ high fat diet induced diabetic nephropathy （DN） rat animal model, after the success of modeling, rats were divided into 4 groups： normal control group, model group, benazepril group and Chinese medicine combined with western medicine group （combination group）. Benazepril 10 mg/kg gavage was given in benazepril group, combination group were treated with Chinese medicine decoction. After 20 weeks the rats were observed the general condition, serum and urine BUN, Cr, TC, TG, 24 h urinary protein, HbAlc and FINS levels, renal tissue were performed after pathological examination, to observe the expression ofTGF-β1、Col-Ⅳ in renal tissue. Results The general condition in combination group obviously improved compared with model group. BUN, Cr, TC, TG, 24 h urinary protein, HbAlc and FINS were significantly decreased in benazepril group and combination group （P 〈0.05, P 〈0.01）. Renal morphological pathology test results： group B had significant lesions, combination group and benazepril group were improved in different degrees. The expression of TGF-β1、Col-Ⅳ in combination group decreased significantly, and compared with model group, the difference was statistically significant （P 〈0.01）. Conclusions The traditional Chinese medicine of invigorating the spleen and tonifying kidney and promoting blood can improve renal damage in type 2 diabetic nephropathy rats, delay the kidney function damage process, which has a protective effect on renal function of DN rats.
