摘  要:目的探讨四川泸州地区人乳头瘤病毒(HPV)多重感染与宫颈病变的关系。方法 2011-2013年在泸州地区各医院收集宫颈病变患者的宫颈脱落细胞或石蜡样本,采用基因芯片技术检测患者HPV感染情况及基因分型。结果 795例宫颈样本中,HPV阳性435例,阳性率为54.7%,HPV阳性率随着宫颈病变程度加重而呈上升趋势(P<0.05)。HPV多重感染比例为42.8%(186/435),其中慢性宫颈炎35.5%(60/169),CINⅠ44.9%(22/49),CINⅡ47.8%(22/46),CINⅢ50.0%(25/50),宫颈乳头瘤样病变45.8%(11/24),宫颈癌47.4%(46/97),不同宫颈病变患者HPV多重感染比例差异无统计学意义(P>0.05)。感染单一高危(低危)亚型HPV与感染多种高危(低危)亚型HPV不同宫颈疾病构成比差异无统计学意义(P>0.05)。结论泸州地区妇女HPV多重感染比例较高。宫颈病变严重程度与感染HPV密切相关,且主要取决于HPV亚型的致病力,与是否为单一或多重感染无关,感染高危亚型HPV需警惕宫颈癌的发生。Objective To investigate the relationship between multiple human papillomavirus (HPV) infection and cervical lesions in Luzhou area of Sichuan. Methods The cervical smear and paraffin tissue samples of 795 women were analyzed byHPV gene microarray technology, the multiple infection status and subtypes of HPV in different cervical lesions were investigated. Results A total of 795 samples were collected in this study, 435 cases were with HPV infections (54.7%). The HPV positive rate showed an ascending trend with the severity of cervical lesions (P〈0.05). Multiple HPV infection rate was 42.8 % ( 186/435). The rate of multiple HPV infections in chronic cervicitis, CINI, CINII, CIN, cervical papilloma-like lesions and cervical cancer were 35.5%(60/169),44.9(22/49),47.8% (22/46),50.0% (25/50), 45.8(11/24),47.4%(46/97), respectively. There was no significant difference in the rate of multiple infections among different cervical lesion groups (P〈 0.05). There was no significant difference in the rate of a single type (high-risk or low-risk) of HPV infection and multiple types (high-risk or low-risk) of HPV infection among all kinds of cervical lesions(P〈0.05). Conclusion The multiple HPV infection rate in Luzhou area is high. The severity of cervical lesions is closely related to infection rate of HPV, depending on the pathogenicity of HPV subtypes rather than quantity of HPV subtypes . More attention should be paid to prevent cervical cancer when infected with high-risk HPV subtypes.
