冠脉介入治疗注射碘对比剂的护理预防和观察

摘　　要：目的分析作好对比剂注射前的护理预防和观察工作对减少副反应的和医患矛盾的作用。方法回顾分析我院2009年1月～2012年12月在本科室住院行冠脉介入治疗的患者3500例,其中男性2000例,女性1500例,年龄35～93岁,平均56岁。选择非离子型含碘对比剂:威士派克32g/100ml;碘帕醇15g/50ml。注射过程中和注射后连续观察患者用药反应。结果在所有的心血管介入患者中,除5例患者有轻度过敏反应出现,无一例出现中度或重度过敏反应,无医疗事故和医患纠纷的发生,远低于同级医院对比剂副反应发生的水平。结论我们的实践表明,作好注射对比剂前的护理预防和观察工作能有效减少副反应的和医患矛盾的发生。Objective To analyze the effect on reducing side effects and patient conflicts by doing care prevention and observation work well before the injection of contrast agent. Methods 3500 cases of patients who were hospitalized in our department in our hospital and performed coronary interventional therapy from January 2009 to December 2012 were retrospecively analyzed, including 2000 men and 1500 women, the age was from 35 to 93 years, mean age was 56 years.The non-ionic iodinated contrast agent was selected： visa pike 32g/100ml, iopamidol 15g/50ml. The drug reaction of patients was observed continuously during injection and after injection. Results Of all cardiovascular interventional therapy patients, no case of moderate or severe allergic reactions occurred, neither medical malpractice nor medical disputes occured, except that mild allergic reactions occurred in the five patients, the level of contrast agent side effects occurred was far below the hospital at the same rank. Conclusion Our practice shows that preparing care prevention and observation work well before the injection of contrast agent could reduce side effects and the occurrence of the doctor-patient conflicts.
