服用缓泻剂对肠瘘患者瘘口愈合干预效果的观察

摘　　要：目的探讨服用缓泻剂对肠瘘患者瘘口愈合干预效果。方法选择肠瘘患者120例，随机分为对照组、试验组（各60例）。实验组肠瘘术后患者在禁食两周后病情稳定窦道形成，可以经口进食补充肠内营养之后，经口服用适量萝卜水、麻仁胶囊、泻叶水、大承气汤（成分：厚朴15～20g，炒莱菔子15—30g，枳壳15g，桃仁9g，赤芍15g，大黄9～15g，芒硝9～15g）等缓泻剂。两组均采用相同的治疗和护理。结果试验组患者瘘口愈合时间明显早于对照组，试验组患者切口感染率明显少于对照组，试验组造口周围粪水性皮炎的发生率明显少于对照组，试验组腹胀发生率明显少于对照组，试验组住院费用明显低于对照组，经比较，两组间数据有差异具有统计学意义，腹部手术切口感染率、造口周围粪水性皮炎发生率，腹胀发生率及瘘口愈合时间差异均有统计学意义。结论采用服用缓泻剂的护理方法，并发症少，瘘口愈合率高，住院时间大大缩短，二次手术可能性大大降低。Objective To analyze the effects of taking laxatives in patients with gastrointestinal fistulas through Intervention therapy. Methods 120 cases of intestinal fistula patients were randomly divided into two groups：control group and experimental group.Each group consists of 60 cases.The condition of patients with fistula in the experimental group was stable after operation for two weeks of sinus formation,and they can be orally fed some supplementary nutrition to meet the need of intestines.Then they can take in some laxative substances,such as radish water,maren capsule,senna leaf water,etc.Two groups use the same treatment and the same care.There was significant difference between the two groups. Results The time of recovery of the patients in the experimental group using fistula healing was faster than that of the control group;the rate of incision infection for the patients in the experimental group was significantly lower than that of the control group;the incidence rate of fecal water dermatitis around the stoma in the experimental group was less than that of the control group;the incidence of abdominal distension was significantly lower in the experimental group than that of the control group; and the hospitalization costs in the experimental group were significantly lower than that of the control group.Compared with the data of the two groups,there have significant differences in the rate of peristomal dermatitis of fecal water,the infection of abdominal incision operation,the abdominal distension and the incidence of fistula healing time. Conclusion The nursing method of taking laxatives is very good and applicable,which leads to less complication,hastens fistula healing time,shortens hospitalization time as well as fees,and reduces the possibility of the second operation.
