急诊患者晕针反应相关影响因素及预防措施分析

摘　　要：目的:分析急诊患者晕针发生的影响因素及预防措施。方法:在我院2011年07月-2012年07月期间急诊采血发生晕针的53名患者为晕针组,每周三门诊采集生化血液标本的第20号、第50号患者共64名为对照组,比较两组患者的年龄、性别、糖尿病史、心脏病史、直立性低血压史、空腹、睡眠、采血时体位、童年打针恐惧经历及采血时不良情绪等情况。结果:单因素分析显示年龄、童年打针恐惧经历及采血时不良情绪与晕针的发生有关(P<0.05);而性别、糖尿病史、心脏病史、直立性低血压史、空腹、睡眠及采血时体位与晕针的发生无关(P>0.05)。多因素Logistic逐步回归分析(后退法)示采血时不良情绪为晕针发生的独立预测因素。晕针组有47人采用抱头扶持法再次采血,均采血成功,其中45人未发生晕针,晕针预防率95.7%,静脉采血成功率100%。结论:晕针与不良情绪(如恐惧、畏痛、心情紧张等)密切相关,而抱头扶持法可以作为一种预防晕针防止意外伤害的有效方法。Objective： To study the influence factors and the pretreatment of acupuncture syncope.Methods：All results of both acupuncture syncope group and control group were obtained during July 2011 to July 2012. The 53 out-patients who had fainted during acupnnctre were tak- en as acupuncture syncope group, while 64 outpatients of the 20th and 50th haemospasia each Wednesday were taken as control group. We compared the difference between the two groups in terms of patients＇ Age, gender, diabetes mellitus, heart disease, orthostatic hypotension, fast- ing state,quality of sleep,body position, scared experience of childhood, unfavorable emotions of heamospasia,etc.Rusuits： We found that acupuncture syncope was associated with patients＇ age, scared experience of childhood and unfavorable emotions of heamospasia by single fac- tors analysis. Further studies by multiple factors logistic regression found that unfavorable emotions of heamospasia is an independent risk factors of acnpuncture syncope. 45 out of 47 acupuncture syncope patients had not fainted again by keeping patients＇ heads weept in other＇s arms with prevention rate 95.7% and success rate 100% .Conclusion：Acupuncture syncope was strongly associated with patients unfavorable emotions of heamospasia and is to be effectively prevented by keeping patients＇ heads weept in other＇ s arms.
