三维多平面显示成像在产前诊断胎儿结构畸形的应用价值

摘  要:目的:探讨三维超声多平面显示模式在产前诊断胎儿结构畸形的应用价值。方法:总结40例产前超声检查时疑似胎儿结构畸形的胎儿,行多平面显示模式进行进一步检查,检查结果均于产后随访、CT、造影或引产后尸检得到证实。分析其检查的价值和技巧。结果:本组40例行产检疑似结构畸形的胎儿中,经产后随访、CT、造影或引产后尸检确诊者38例,误诊2例为没发生积水的重复肾者。其中确诊腭裂诊断20例,心脏结构畸形12例,胼胝体缺如3例,小脑蚓部缺失2例,马蹄肾1例。结论:产前超声诊断胎儿结构畸形,尤其是在有些二维影像难以显示的结构时,三维多平面成像可以提供更有价值的超声影像,增加超声医生的诊断信心[1,2]。Objective: To investigate the relevant value oi three-dimensional (3D) multi-slice view (MSV) mode of ultrasonography on fetal structural malformations when prenatal diagnosis. Methods: 40 fetuses, which were considered as suspicious fetal structural malformations after prenatal ultrasound examination, were selected for further analysis by the multi-slice view mode. The inspection result was proved by CT or radiography after born or necropsy after induced labor. Analysis was performed on the value and skill of 3D-MSV mode examination. Results: Among 40 suspicious cases, 38 fetuses were diagnosed fetal structural malformations. The other two misapplied cases were those with duplication of kidney without hydronephrosis. 50% (20/40) cases were diagnosed blade edge, 30% (12/40) were diagnosed heart defects, 7.5% (3/40) absence of callus, 5% (2/40) cauda cerebelli, 2.5% (1/40) horseshoe kidney. Conclusion: 3D-MSV mode provided more valuable ultrasonic image and strengthen the confidence of doctors, especially when tile structure was hardly displayed by bidimensional image.
