PKP和PVP结合过伸复位法治疗骨质疏松性椎体压缩骨折的对比研究

摘　　要：目的：探讨PKP或PVP结合过伸复位法骨质疏松性压缩骨折中的复位作用，对两者的作用和效果进行对比。方法：对2007年11月～2012年10月间行PKP的21例患者和行PVP的45例患者进行随访，分别记录每个病例手术前后疼痛程度（ VAS评分）、手术前后椎体复位情况、随访终点时椎体高度丢失情况（以Cobb角来测量），并将两组病例进行对比研究。结果：平均随访时间为12月。结果显示两组病例VAS评分、椎体复位情况及Cobb角均较术前明显改善。两组间在VAS评分方面无明显差别，但PKP具有更好地复位和改善椎体后突畸形效果。结论：PKP和PVP均能减轻疼痛，增加椎体强度。 PKP能更好地复位压缩椎体，恢复椎体高度和改善椎体后突畸形。Objective：To assess the clinical effects of percutaneous ky-phoplasty （ PKP ） and percutaneous vertebroplasty （ PVP ） combined with hyperextensive reduction in treatment of osteoporotic vertebral compressive fractures（OVCF）.Methods：Between Nov.2007 and Oct.2012,66 pa-tients（66 vertebrae）with OVCF underwent PKP（n =21）and PVP（n =45）combined with hyperextensive reduction assisted by C-arm fluorosco-py.The data were kept and compared in the two groups of patients regard-ing pain degree before and after operation by Visual Analogue Scale （ VAS） ,postoperative restoration of the height of the vertebral body and loss of disk height and correction of the kyphotic angle by Cobb angle measurement by the end-point of follow-up.Results：Postoperative follow-up ranging from 6 to 36 months（ mean 12 months） showed that the two groups of patients were significantly improved postoperatively concerning the VAS scores,restoration of the vertebral body and correction of the ky-photic angle.Although the two groups had no significant difference in VAS scoring,yet PKP had led to better restoration of the vertebral body and kyphotic deformity.Conclusion：Both PKP and PVP can reduce the pain for patients with OVCF and improve the strength of the vertebral body,and the former appears superior to the latter in restoration of the ver-tebral body and correction of the kyphotic deformity .
