成人腹腔镜腹股沟疝修补术131例

摘　　要：目的总结成人腹腔镜腹股沟疝修补术（LIHR）的效果及经验.方法 回顾性分析安徽医科大学第三附属医院2011年6月至2013年1月收治的成人腹腔镜腹股沟疝修补术131例患者的临床资料.经腹腹膜前疝修补术（TAPP）59例,完全腹膜外修补术（TEP） 72例,其中3例为TEP中转TAPP.结果本组131例患者手术均获得成功,手术时间为44～107 mins,平均（62±11）min;住院时间3～8 d,平均（6.2±1.2）d;术中出血量15～35 ml,平均（28.2±3.5）ml;术后并发症发生率为7.6%（10/131）,其中腹股沟区血清肿3.8%（5/131）,腹股沟区疼痛2.3%（3/131）,术后尿潴留1.5%（2/131）,无一例感染,随访2～21个月,平均随访10个月,无一例复发.结论 LIHR安全有效,并发症少,复发率低,在熟练掌握其技术后,是成人腹股沟疝治疗的趋势.Objective To summarize the effectiveness and the experience of laparoscopic inguinal hernia repair in adult patients. Methods A total of 131 cases with inguinal hernias were enrolled and treated by laparoscopic inguinal hernia repair from June 2011 to January 2013 in No. 3 Hospital Affiliated to Anhui Medical University. The patients were treated by transabdominal preperitoneal prosthesis （TAPP） in 59 cases and by totally extraperitoneal prosthesis （TEP） in 72 cases, including 3 cases of TEP who were converted to TAPP. Results All the cases of laparoscopic inguinal hernia repair were successful. The operation time lasted 44 to 107 rains with a mean of （62 ~ 11 ） min. The length of hospital stay is 3 to 8 days with a mean of （6.2 ~ 1.2） days, and intraoperative blood loss was 15 to 35 ml with a mean of （28.2 ＋ 3.5） ml. The percentage of postoperative complications was 7. 6% （10/131）, including inguinal area seroma which accounted for 3.8% （5/131）, 2.3% （3/131） of inguinal region pain, and 1.5% （2/131） of postoperative urinary retention. There was no case of infection in the follow-up of 2 to 21 months （a mean of 10 months）, and no case of recurrence was observed. Conclusion LIHR is safe, effective with fewer complications and low recurrence rate.
