摘  要:目的钡餐摄片观察食管癌大体肿瘤靶区(GTV)呼吸移动度。方法 30例食管癌患者分为三组:A组11例,肿瘤位于颈段和胸上段;B组10例,肿瘤位于胸中段;C组9例,肿瘤位于胸下段。口服钡餐摄片获得平静呼吸状态下吸气相、呼气相GTV图像,通过Eleka-iViewGT工作站比对,得出各组GTV在X、Y、Z轴方向的位移值。结果 GTV图像在X、Y、Z轴方向位移值:A组为(1.09±0.77)mm、(1.37±0.67)mm、(1.52±0.64)mm;B组为(1.84±1.06)mm、(2.11±1.28)mm、(3.07±1.60)mm;C组为(3.00±1.41)mm、(4.50±1.46)mm、(7.46±1.65)mm。A、B组在Y、Z轴方向位移值均小于C组(P<0.01);B组在Z轴方向位移值大于A组(P<0.05)。结论钡餐摄片可动态观察食管癌GTV移动度,临床可用于确定内靶区外扩范围。Objective To observe the breath-induced target motion degree of gross tumor volume(GTV) in esophageal carcinoma by taking barium meal film. Methods Thirty patients with esophageal carcinoma were divided into three groups of A (cervical segment and upper thoracic portion, 10 cases), B(mid-thoracic portion, 10 cases) and C (lower thoracic portion, 10 cases). The displacement values of breath-induced target motion on the X, Y and Z axis were obtained by comparing GTV images with Eleka-iViewGT workstation. Results Displacement values of GTV images on the X,Y and Z axis were (1. 092±0.77) mm, (1.37±0.67) mm and (1.52±0.64) mm in group A,(1.84 ±1.06) mm, (2. 11 ± 1.28) mm and (3. 07 ± 1.60) mm in group B, (3.00±1.41) mm, (4. 50±1.46) mm and (7.46±1.65) mm in group C. Displacement values on the Y and Z axis in groups of A and B were less than those in group C(P〈0. 01). Displacement value on the Z axis in group B was greater than that in group A(P〈0. 05). Conclusion The breath-induced target motion degree of GTV for esophageal carcinoma can be dynamically observed by X-ray barium meal film, by which extended range of internal target volume may be determined clinically.
