摘  要:目的探讨双源CT心电门控下冠状动脉成像对临床拟诊冠心病患者升主动脉弹性评估的价值及其与冠状动脉狭窄程度之间的相关性。方法搜集2011年5月至2012年3月间我院临床拟诊冠心病并行双源CT冠状动脉造影患者中图像质量好、无呼吸及心律伪影的121例纳入本研究(男73例,女48例);分为冠状动脉狭窄组和冠状动脉正常对照组。以R波与T波顶点时相重建,获得主动脉瓣完全关闭及完全开放图像,并计算同一层面升主动脉收缩期及舒张期直径和横断面积。同时行冠状动脉狭窄评分(CAS)。结果升主动脉弹性指标[相对直径变化(%Ao)、膨胀性(AoD)、顺应性(AoC)及僵硬度(AoSI)]在冠状动脉狭窄组与对照组间差异具有统计学意义(P<0.05)。冠状动脉不同狭窄程度组间的弹性指标差异具有统计学意义(P<0.05)。升主动脉弹性指标(%Ao、AoD、AoSI)与冠状动脉狭窄评分具有显著相关性(P<0.01)。结论升主动脉弹性与冠状动脉狭窄程度密切相关,也是心血管疾病的危险因素之一,可作为冠心病的独立预测因子。Objective To evaluate the correlation between ascending aortic elasticity and the degree of coronary steno- sis using DSCT. Methods 121 adults (73 males and 48 females) with coronary heart disease underwent DSCT angiogra- phy examination. The patients were divided into stenosis group and normal group. Rebuild the image of T wave vertex and R wave vertex in the whole cardiac cycle to obtain systolic and diastolic diameter and area. Results Stiffness and expan- sion degree in the ascending aorta had statistically significant differences between coronary heart disease group and the con- trol group. There were significant differences among the different stenosis degree subgroups in aortic elastic index. There were significantly negative correlation between % AO, AoD and Gensini score, and significantly positive correlations be- tween AoSI and Gensini score. Conclusion Arterial elastic index is closely related to the coronary artery stenosis degree, which is the risk factor for cardiovascular disease. It can be used as prediction index of coronary artery lesions stenosis de- gree.
