摘  要:目的探讨采用后外侧减压、椎间融合结合钉棒系统前后一体固定技术治疗胸腰段椎间盘突出症的临床效果。方法总结21例胸腰段椎间盘突出症患者,采用后外侧减压、椎间融合结合钉棒系统前后一体固定技术治疗。结果平均随访13.3个月,21例患者术后疼痛均有明显改善,神经功能有1级或1级以上的改善,1例出现胸腔积液,经胸腔穿刺、引流后消失;无切口感染,随访期内无断钉、断棒、螺钉松动等现象。结论后外侧减压、椎间融合结合钉棒系统前后一体固定技术,经后外侧同一切口实现了有效椎管减压的同时,通过椎弓根钉及椎体钉的一体固定,重建了脊柱的稳定性,具有明显的优点。Objective To investigate the clinical effect of posterolateral decompression,interbody fusion combined with one nail rod system fixation for the treatment of thoracolumbar intervertebral disc herniation. Methods To summarize the clinical data of 21 patients with thoracolumbar intervertebral disc herniation,who received posterolateral decompression, interbody fusion combined with pedicle screw system fixation and integration. Results The patients were followed up for 13.3 months,the postoperative pain was significantly improved in 21 cases,nerve function had 1 or more than 1 grade improvement. Pleural effusion occurred in 1 case,and disappeared after thoracic puncture, drainage. No wound infection observed. No broken nails,broken rod, screw loosening observed during follow-up. Conclusion Posterolateral decompression,interbody fusion combined with pedicle screw system before and after the integration fixation through posterior lateral incision, the same to realize effective decompression of the spinal canal at the same time, through the pedicle screw and vertebral nail one fixed, reconstruction of spine stability,has obvious advantages.
