代谢综合征患者的动态血压特点分析

摘　　要：目的观察代谢综合征(MS)患者的血压参数,探讨MS患者的血压分布规律。方法采用日本TM2420/T型动态血压监测仪分别监测50例MS患者、50例单纯的高血压患者及30例健康人的24 h动态血压,比较3组间平均血压、血压负荷、血压昼夜节律及血压晨峰。结果健康人平均血压、血压负荷正常,血压变化曲线呈杓形,血压晨峰正常;高血压患者平均血压水平升高,血压负荷增加,但血压波动曲线与健康人相似,血压晨峰异常患者增加;MS患者平均血压水平及血压负荷增高,特别是夜间升高明显,昼夜节律消失,呈非杓形,血压晨峰异常比例较高血压患者增加。结论 MS患者较单纯的高血压患者心血管危险更大,MS患者的降压治疗应优先考虑能使患者血压更符合正常波动曲线的长效制剂。Objective It is to observe blood pressure parameter of patients with metabolism syndrome （MS） , and ap proach the regularities of blood pressure distribution. Methods The 24 - hour blood pressure of 50 MS cases, 50 simple hyper tension cases and 30 healthy people were monitored by type TM2420/T Ambulatory Blood Pressure Monitor made in Japan, the mean blood pressure value, blood pressure load, the day and night rhythm of blood pressure and the morning blood pressure surge between three groups were compared. Results The value of mean blood pressure and blood pressure load were normal, the blood pressure fluctuation curve showed spoon, and the morning blood pressure surge was normal in healthy control group. The values of mean blood pressure, blood pressure load were generally raised in simple hypertension group, but the blood pres sure fluctuation urve showed similar to that of control group. The number of patients with raised morning blood pressure surge was higher than that of control group. The value of mean blood pressure and blood pressure load were raised in MS group, es pecially during night, and the day and night rhythm of blood pressure were disappeared, the blood pressure fluctuation curve was showed non-spoon, and the number of patients with raised morning blood pressure surge was higher than that of simple hy pertension group. Conclusion Patients with MS have more cardiovascular risks than patients with simple hypertension, it is more reasonable to select prolonged-action drug preparations that are fit for blood pressure fluctuation curve for patients with MS.
