摘 要:目的研究冠心病不稳定型心绞痛气滞血瘀证、痰浊痹阻证患者的代谢组学特征,探讨代谢组学方法在研究不稳定型心绞痛中医证型中的应用。方法对12例不稳定型心绞痛气滞血瘀证患者和12例不稳定型心绞痛痰浊痹阻证患者的血浆样本进行氢核磁共振(1H NMR)检测。通过偏最小二乘判别分析(PLS-DA)方法,研究不稳定型心绞痛气滞血瘀证患者与痰浊痹阻证患者之间血浆代谢产物谱及的差异。结果各组血浆核磁共振氢谱的PCA结果显示,不稳定型心绞痛气滞血瘀证患者与不稳定型心绞痛痰浊痹阻患者能够被区分,两组血浆样本之间的代谢物含量存在明显差异。不稳定型心绞痛气滞血瘀证组血浆中丙氨酸、高密度脂蛋白、不饱和脂肪酸、瓜氨酸、精氨酸、脯氨酸的含量高于痰浊痹阻证组,而低密度脂蛋白、极低密度脂蛋白、脂类化合物、酮体、乳酸、葡萄糖、天门冬氨酸、谷氨酞胺的含量低于痰浊痹阻证组。结论两组血浆样本的1H NMR代谢谱存在差异,能从代谢组学分析中找出特异的标志性代谢产物。血浆代谢物变化能在一定程度上区分冠心病不稳定型心绞痛的这两种中医证型。Objective To explore the plasma metabonomic profile of unstable angina peetoris patients with Qi-stagnation and blood-stasis syndrome(QBS) and with phlegm-turbid syndrome (PS). To examine potential application of metabonomic analysis in studies of Chinese Traditional Medicine syndromes of Unstable Angina Peetoris patients. Methods Totally 12 cases of unstable angina pectoris patients with Qi-stagnation and blood-stasis syndrome (QBS) and 12 cases unstable angina pectoris patients with phlegm-turbid syndrome (PS) were enrolled in this study and their plasma metabolites were analyzed using Hydro-Nuclear Magnetic Resonance (1H NMR)spectrometer for the Partial Least Squares Discriminant Analysis(PLS-DA). Results PLS-DA of plasma ^1H NMR spectra revealed different metabolic spectra between QBS patients and PS patients, demonstrated that the metabolic characteristics of 2 groups were significantly different. Compared with PS patients, alanine, high-density lipoprotein, unsaturated fatty acids, citrulline, arginine, proline increased in QBS patients, while low- density lipoprotein, very low density lipoprotein, lipid compounds, ketone bodies, lactic acid, glucose, aspartate, thalidomide glutamyl decreased considerably. Conclusion Characteristic metabolic products in two groups can be identified by ^1H NMR based metabonomies analysis. Metabonomic study is a feasible and promising way to detect the variation of plasma etabonomics of Unstable Angina Pectoris patients with different TCM Syndromes.
