摘  要:目的榆测包虫病灶切除术前、术后Th17相关细胞因子(IL-17、IL-23)的表达,探明包虫病灶切除术前、术后免疫状态变化情况。方法将SD大鼠分为健康对照组、泡球蚴感染对照组和泡球蚴病手术组(简称手术组),分别于腹股沟注射感染泡球蚴(健康对照组用中理盐水代替),3月后将感染成功的大鼠纳入试验。对手术组大鼠切除皮下包虫病灶,术后第1~7d各处死大鼠6只,第7d处死健康对照组和泡球蚴感染对照组大鼠各6只,采集下腔静脉血液和肝脏组纵,分别用ELISA法检测血清和肝匀浆组织IL-17和IL-23水平,用Real—timeRT—PCR检测肝脏IL-17mRNA和IL-23mRNA表达水平。结果包虫病大鼠血清IL-17和IL-23分别为(8.38±1.04)pg/ml和(352.69±25.04)pg/ml,健康对照组分别为(17.14±1.21)pg/ml和(450.05±29.37)pg/ml,差异有统计学意义(P〈0.05);手术组术后分别于第2d和第3d开始增高,于第6d和第7d开始趋于正常。包虫病大鼠肝匀浆IIL-17和IL-23分别为(7.68±2.21)pg/ml和(336.65±28.73)pg/ml,健康对照组分别为(16.88±2.36)pg/ml和(446.68±24.53)pg/ml,差异有统计学意义(P〈0.05):于术组术后分别于第3d和第4d开始增高,IL-17于第7d趋于正常,IL23第7d仍低于健康对照组。包虫病大鼠肝IL-17mRNA和IL-23mRNA分别为(0.32±0.08)pg/ml和(0.96±0.79)pg/ml,健康对照组分别为(0.73±0.97)pg/ml和(1.57±0.73)pg/ml,羞异有统计学意义(P〈0.05);手术组术后分别于第1d和第3d开始增高.于第4d和第6d开始趋于正常。结论IL-17和IL-23为Th17细胞发挥功能的主要细胞因子。包虫病灶切除后,包虫寄生所致的IL-17和IL-23抑制状态能存较短时间内恢复并趋于正常,进而可能对其他感染或者移植器官产生趋于日常的免疫排斥反应。Objective To determine the expression of Th17 cytokines (1L-17 and IL-23) before and after resection of hydatid lesions and ascertain postoperative changes in immune status. Methods SD rats were divided into a healthy control group, a control group infected with alveolar hydatids, and a group that underwent surgery to resect alveolar hydatid lesions. Rats were infected in the groin by injection of alveolar hydatids (the healthy control group received saline instead). Successfully infected rats were included in this study after 3 months. Subcutaneous hydatid lesions were removed from rats that underwent resection. Six rats were sacrificed on day 1 to 7 postoperatively. Six rats from the healthy control group and 6 from the control group infected with alveolar hydatids were sacrificed on day 7. Blood from the inferior vena cava and liver tissue were collected. Levels of IL-17 and IL-23 expression in serum and liver tissue were measured with ELISA, and IL-17 mRNA and II. 2,3 mRNA were detected using real-time RT-PCR. Results Serum levels of IL-17 and IL-23 were 8. 38± 1.04 pg/ml and 352.69 ± 25.04 pg/mlin rats with echinococcosis compared to levels of 17. 14± 1.21 pg/ml and 450.05 ±29.37 pg/ml in the healthy control group; differences in the serum levels of IL-17 and IL-28 were statistically significant (P 〈 0.05). After surgery, levels of IL-17 and IL-Rg began to increase on day 2 and day 3 and those levels tended to be normal on day 6 and day 7. Levels of IL-17 and IL 23 in liver homogenate from rats with echinococeosis were lower than those in the healthy control group. After surgery, levels of IL-17 and IL 23 begum to increase on day 3 and day 4. IL-17 tended to be normal on day 7. The level of IL-23 was still significantly lower than that in the healthy control group on day 7. Levels of IL 17 and IL-23 in liver homogenate from rats with echinococcosis were 7.68 ± 2.21 pg/ml and 336.65 ± 28.73 pg/ml compared to levels in the heahhy control group of 16.88± 2.36 pg/ml and 446.68 ± 24.53 pg/ml
