腹腔镜高位宫骶骨韧带缩短悬吊治疗子宫脱垂50例临床分析

摘　　要：目的探讨腹腔镜下高位宫骶韧带缩短悬吊术治疗子宫脱垂的可行性与安全性。方法对2009年1月至2011年12月在佛山市第一人民医院50例子宫脱垂的患者行腹腔镜下高位宫骶韧带悬吊术，对比手术前及术后最后一次复诊POP-Q评分，应用描述性统计及配对t检验评估手术效果，就患者的主观满意度及性生活质量进行随访问卷调查。结果随访12～36月，经统计学分析手术前后c、D、Aa、Ba、Ap、Bp点经配对t检验，差异有统计学意义（P〈0．001）。5例（10％）复发，有效率90％。患者主观满意度100％，性生活质量明显改善。结论腹腔镜高位宫骶韧带悬吊术治疗子宫脱垂是有效的、安全的、微创的，可明显改善患者的生活质量。但对于伴有长期腹压增加的患者，需慎重选择该术式。Objective To assess the safety and feasibility of laparoscopic high uterosacral ligament uterine suspension for the treatment of uterine prolapse. Methods 50 cases of uterine prolapse patients were carried out laparoscopic high uterosacral ligament uterine suspension from January 2009 to December 2011, and their preoperative and postoperative POP-Q rating were compared with last examination to evaluate the operation outcomes by applicating the descriptive statistics and the paired t-test. At same time, to investigate the patient＇s subjective satisfaction and sexual life quality by questionnaire survey. Result All followed up for 12 to 36 months , and after statistics analysis by paired t-test pro-operate and post-operate C, D, Aa, Ba , Ap, Bp points, differences are statistically significant （P 〈 0. 001 ）, There were 5 recurrence cases, and the effective rate is 90%. Patients＇ subjective satisfaction was 100%, and the sexual life quality was improve notably. Conclusion Laparoscopic high uterosacral ligament uterine suspension appears to be a safe, micro invasive and effective treatment option in short term for uterine prolapse patients, it can obviously improve the patient＇s quality of life. However, patients associated with an increase in abdominal pressure for a long time need to carefully selectthis operation.
