3443例体检女性宫颈高危型人乳头瘤病毒感染状况分析

摘　　要：目的了解西部地区健康女性宫颈高危型HPV的感染情况、年龄分布特点及其与宫颈细胞学之间的关系。方法前瞻性分析2010～2011年到华西医院体检女性的宫颈高危型HPV检测及细胞学结果。结果本研究共纳入研究对象3443名，总体阳性率为21．49％。其中，60（含）-65岁年龄段阳性率最高，阳性率为28．57％，除阳性率最低的70岁（含）以上年龄段外，30岁以下的年龄段最低（P〈0．05）。在702名高危型HPV感染者中，细胞学异常者为2．14％，与HPV感染阴性者比较，两者间差异有统计学意义（P〈0．05）。结论正常人群中，60（含）～65岁年龄段的女性是HPV感染的高峰人群，宫颈高危型HPV的感染较前有年轻化的趋势，感染率有增高的趋势，感染后并不一定会出现宫颈上皮内瘤变。HPV检测在协助分析不正常的细胞学结果中，具有重要的临床意义。OBJECTIVE The study aimed to assess the prevalence of infection of cervical high-risk papillomavirus （HR- HPV） , the characteristics of the age distribution, and the correlation between HR-HPV infection and cervical cytology a- mong healthy females in west China. METHODS Results on HR-HPV testing and cervical cytology among female physical examinees in West China Hospital during 2010 and 2011 were prospectively analyzed. RESULTS The study included 3443 research subjects, with an overall HPV positive rate of 21.49%. Subjects aging between 60 and 65 had the highest positive rate （28.57%） and those younger than 30 had the lowest positive rate （P〈 0.05）, if excluding the group over the age of 70, which showed the lowest positive rate of all groups. 2.14% of the 702 high-risk HPV-positive cases exhibited abnormal cytol- ogy, which was significantly higher than that of the HPV-negative cases （ P 〈 0.05）. CONCLUSION Women between the age of 60 and 65 have the highest HPV infection rate among all other age groups ; while a rising trend of HR-HPV infection is observed among younger women. However, HR-HPV infection is not necessarily accompanied by the occurrence of cervical intraepithelial neoplasia; therefore, HR-HPV test plays an important role in the analysis of abnormal cytology test results and clinical diagnosis of the disease.
