摘 要:目的探讨血清Ghrelin水平与多囊卵巢综合征(PCOS)胰岛素抵抗的相关性。方法 72例PCOS患者根据体重指数(BMI)随机分为肥胖组(A组,35例)和非肥胖组(B组,37例),同时选择70例健康体检者作为对照组,其中肥胖组(C组,34例)和非肥胖组(D组,36例)。检测4组空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)和血清Ghrelin水平。结果 PCOS患者FINS、HOMA-IR较对照组明显增高(P﹤0.05),而血清Ghrelin水平明显降低(P﹤0.05);PCOS患者和对照组中,肥胖组FINS、HOMA-IR较非肥胖组明显增高(P﹤0.05),而血清Ghrelin水平明显降低(P﹤0.05);PCOS患者血清Ghrelin水平与体重指数(BMI)、FINS和HOMA-IR呈显著负相关性(r=-0.587,P=0.006;r=-0.237,P=0.021;r=-0.412,P=0.003)。结论 PCOS患者血清Ghrelin水平较低,与胰岛素抵抗呈负相关,其可能在PCOS的病理生理过程中发挥重要作用。OBJECTIVE The aim of this study was to investigate the correlation between serum ghrelin level and insulin resistance in polycystic ovary syndrome (PCOS). METHODS 72 PCOS patients were randomly divided according to their body mass indexes (BMIs) into an obese group (group A, n = 35) and a non-obese group (group B, 37 cases) ; 70 healthy individuals were selected as the control group and were divided into an obese group (group C, n = 34) and a non-obese group (group D, 36 cases). Levels of fasting blood glucose (FBG) , fasting insulin (FINS) , insulin resistance index (HOMA-IR), and serum ghrelin in the 4 groups were determined. RESULTS The PCOS patients showed significantly higher FINS and HOMA-IR and significantly lower serum ghrelin level than the control group (P 〈 0.05 for both cases). FINS and HOMA-IR of the obese groups in both the PCOS patients and the controls were significantly higher than those of the nonobese groups (P 〈 0.05), and serum ghrelin levels of the Obese groups were significantly lower (P 〈 0.05). Serum ghrelin lev- el in the PCOS patients exhibited a significantly negative correlation with BMI, FINS, and HOMA-IR (r = -0.587, P = 0.006; r = -0.237, P = 0.021; r = -0.412, P = 0.003). CONCLUSION Patients with PCOS generally exhibit low levels of serum ghrelin, which is shown to negatively correlate with insulin resistance. Therefore, serum ghrelin level may play an important role in the pathophysiological process of PCOS.
