腰大肌横断面积与腰椎间盘突出的关系

摘　　要：目的 :测量腰大肌横断面积 ,探讨其与腰椎间盘突出的关系。方法 :随机选择 14 0例腰椎间盘突出病人和 72例正常对照组 ,在L4～ 5和L5～S1间隙水平测量双侧腰大肌横断面积。结果 :①正常腰大肌横断面积 (L4～ 5:男 15 .0 3cm2 ,女 8.6 0cm2 ;L5～S1:男 14 .42cm2 ,女 8.39cm2 ) ,男性较女性明显增大 ,但双侧腰大肌之间无显著差异。②双侧腰大肌横断面积的差值 ,腰椎间盘突出症组高于对照组。③男性组中的腰大肌横断面积 ,腰椎间盘突出组较正常对照组减小。结论 :双侧腰大肌不对称及腰大肌萎缩变细可能均是腰椎间盘突出的解剖因素之一。Objective: To Study the relationship between the transverse area of the psoas muscle and lumbar disc herniation.Methods: The transverse area of the psoas muscles were measured at the level of L 4～5 and L 5～S 1 in 140 randomly selected lumbar disc herniation patients and 72 healthy volunteers.Results: ①The transverse area of psoas muscle is significantly larger in male than in female, but it has not significant difference between right and left in healthy volunteer group.②The difference between right and left transverse area of the psoas muscle is significantly bigger in the lumbar disc herniation patient group than in the healthy volunteer group.③The transverse area of the psoas muscle in male is significantly smaller in the lumbar disc herniation patient group than in the healthy volunteer group.Conclusion: Both the asymmetry between right and left psoas muscles and the amyotrophy or attenuation of the psoas muscle are probably the anatomical factors which lead to lumbar disc herniation.
