摘  要:目的:探讨神经电图对糖尿病周围神经病亚临床病例早期诊断的临床应用价值。方法选择我院2011年1月至2012年12月2型糖尿病患者68例作为观察组;并选择同期来我院体检的健康人群66例作为对照组。采用肌电图仪分别对两组患者行神经传导速度(NCV)、胫神经H反射、胫神经F波检测。结果:在观察组68例糖尿病患者中,临床诊断糖尿病周围神经病51例(75%),经电生理检查后增至60例(88.24%),确诊率由73.52%上升到88.24%;17例无周围神经病变症状的患者中,电生理检查后确诊异常9例,异常率占52.94%,两组运动神经传导速度(MCV)和感觉神经传导速度(SCV)均存在显著性差异(P<0.01)。观察组胫神经F波异常率50%,H反射异常率69.9%,两组H反射异常率和F波异常率差异有统计学意义(P<0.01)。结论:NCV、F波及H反射是早期发现糖尿病周围神经病变的有效手段,且H反射是最敏感的指标,在糖尿病周围神经病亚临床病例早期诊断中具有临床应用价值,为糖尿病周围神经病临床疗效评估及治疗方案拟定提供重要依据。Objective :To explore neural electrical diagram for diabetic peripheral neuropathy and clinical value in early diagnosis of clinical cases .Methods :Select our hospital from January 2011 - December 2012 68 cases of type 2 diabetes patients as observation group ;And select at the same time to the hospital physical examination of 66 cases of healthy people as control group .Using emg meter ,respectively on two groups of patients with nerve conduction velocity (NCV) ,tibial H reflex ,tibial F wave detection .Results :In observation group ,68 cases of diabetic patients ,the clinical diagnosis of diabetic peripheral neuropathy (75% ) ,51 patients by electrophysiological examination after increased to 60 cases (88 .24% ) ,positive rate from 73 .52% to 88 .24% ;17 cases of patients with no symptoms of peripheral neuropathy ,electrophysiological examination diagnose abnormal after 9 cases ,the abnormal rate was 52 .94% ,two groups of motor nerve conduction velocity (MCV) and sensory nerve conduction velocity (SCV) there were significant differences (P 〈 0 .01) .Observation group tibial F wave abnormal rate was 50% ,H reflex abnormal rate is 69 .9% ,two groups of H reflex abnormal rate and F wave abnormal rate difference was statistically significant (P 〈 0 .01) .Conclusion :NCV、F wave and H reflex is an effective method for early detection of diabetic peripheral neuropathy ,and H reflection is the most sensitive indicator of the clinical cases in early diagnosis of diabetic peripheral neuropathy has clinical application value ,clinical curative effect evaluation and treatment of diabetic peripheral neuropathy can provide important basis .
