摘 要:目的制备载铁的液固相变型聚乳酸-羟基乙酸(PLGA)原位植入物(Fe-ISFI),初步探讨其在超高频交变磁场下的产热性能及对离体猪肝的消融效果。方法制备3种含铁量分别为30%、40%、50%的Fe-ISFI,以超声观察其液固相变过程,扫描电镜(SEM)观察其相变后的内部结构与形态学变化。将上述3种Fe-ISFI各100μl分别注入离体猪肝内,行微型CT(Micro-CT)及超声检查。将液固相变后的Fe-ISFI置于交变磁场中加热,用远红外测温仪观测Fe-ISFI温度随时间的变化。选取最优Fe质量分数的Fe-ISFI 100μl注入离体猪肝内,以交变磁场加热,观测注射处肝表面温度随时间的变化,并于该处温度达60℃时记录距制剂注射处周围1、2、3、4、5cm处的温度;取肝组织内的Fe-ISFI行SEM观察,对肝组织行HE染色及病理观察。结果 Fe-ISFI液固相变后呈强回声,内部回声不均质。Fe-ISFI在加热前内部呈较大蜂窝状,加热后蜂窝变小,表面变光滑。猪肝内的铁含量为50%的Fe-ISFI形态最规则。随着时间延长,Fe-ISFI温度逐渐增高,以50%的Fe-ISFI增高最明显。Fe-ISFI在肝组织内的磁加热效果与体外相似,Fe-ISFI周围组织温度随距离递减。加热后Fe-ISFI周围肝细胞发生凝固性坏死。结论在超高频交变磁场作用下,Fe-ISFI能在短时间内产生明显的热效应,有望用于磁热治疗肿瘤。Objective To prepare Fe-loaded liquid-solid phase inversion poly lactic-co-glycolic acid (PLGA) implants (Fe-ISFI), and to explore its potential property on magnetic hyperthermia and the ablation efficiency on isolated pork liver. Methods Three different filler fractions of Fe in Fe-ISFI (30wt-%, 40wt-%, 50wt-%) were prepared. The process of liquid-solid phase inversion and the inner structure of the solids were observed with ultrasound and scan electron microscope (SEM), respectively. In addition, 100 μl of the above three different Fe-ISFI solution were injected into the isolated pork liver tissue respectively, then micro CT (Micro-CT) and ultrasound examination were performed. The solidified Fe-ISFI in the alternating magnetic field was heated, and temperature vs. time was recorded, then the optimum filler fraction Fe-ISFI was chosen. In isolated pork liver tissue, 100 μl of the chosen Fe-ISFI was injected and also heated in the magnetic field, of which temperature vs. time of the liver surface above the injection point and temperature of the vicinity were recorded. After heating, the solidified Fe-ISFI was sent for SEM and the isolated pork liver tissue was fixed by 4% paraformaldehyde for HE stain. Results The solidified Fe-ISFI was hyperechogenic in ultrasonography and cellular inside showed by SEM. The injected 50% Fe-ISFI inside the liver tissue had the best morphology. The temperature increased with time, and 50% Fe-ISFI increased most quickly. The same heating efficiency was approved in isolated pork liver tissue and temperature decreased with distance. Coagulation necrosis of hepatic cells around the Fe-ISFI were observed under microscopy. Conclusion Fe-ISFI in the alternating magnetic field is able to get high temperature quickly, having potential for magnetic thermal therapy of tumor.
