摘 要:目的应用(speckle tracking imaging,STI)技术评价新生猪模型受损心肌纵向运动,观察心肌组织病理变化。方法 18只新生约克猪随机分成对照组(n=8)和模型组(n=10),模型组行气管插管、低氧通气及双侧颈动脉夹闭术,对照组不予低氧及夹闭。造模24 h行超声心动图检查,获取左室各节段纵向应变及应变率曲线,测量曲线峰值,取各水平平均值。测量心率、LVEF及Tei指数。测量二尖瓣环TDI频谱Sa、Ea、Aa峰值平均值。对脑组织及心肌组织行病理学检测。结果与对照组相比,模型组各水平SL、SrL S、SrL E、SrL A减低,HR增快,LVEF下降,Tei指数升高,二尖瓣环Sa、Ea、Aa峰值减低,P<0.05,差异有统计学意义。模型组心肌细胞肿胀、变性、坏死及凋亡;脑神经胶质细胞肿胀、神经元细胞变性及坏死。结论新生猪HIE模型可复制新生儿HIE并发心肌损害,模型早期即出现心肌纵向运动及心功能减弱。Objective To observe the changes of longitudinal motion by speckle tracking imaging(STI) and the pathologic of the damaged myocardium of newborn pigs with hypoxic ischemic encephalopathy. Methods 18 newborn pigs were randomly divided into control group (n=8) and model group (n=10). The piglets were blocked bilateral ca rotid artery and given 6% oxygen mechanically in the model group. After 24 hours, the longitudinal strain(SL), longitudinal strain rate(SrI.) S, SrL E and SrL A of each segment were measured by STI. Then the average of global, base, middle and apex levels were calculated. And the heart rate, LVEF, Tei index and the peak value of TDI spec trum of the mitral annulus were measured. The pathological slices of the myocardium and brain were observed at last. Results Compared with the control group, the SL, SrL S, SrL E, SrL A, LVEF, Sa, Ea and Aa decreased, but the HR and Tel index increased in the model group (P〈0.05). In the model group, swelling, degeneration, necrosis and apoptosis were found in some myocardial cells. Also, swelling, degeneration and necrosis were found in some brain glial cells and neuronal cells in the model group. Conclusions It is a reliable method to use newborn pigs model with HIE to simulate neonatal hypoxic ischemic encephalopathy(HIE) with myocardial damage. The longitudinal motion and heart function decreased in the early time of the HIE model.
