摘  要:目的:探讨多层螺旋CT(MSCT)多平面重组(MPR)技术在腹腔巨大囊性病变的诊断价值。方法:回顾性分析28例经临床手术病理证实的A、B、C 3组腹腔巨大囊性病变的CT影像,A组为螺旋CT横断面影像,B组为MPR(multiple planar reconstruction,MPR)影像,C组为螺旋CT横断面并辅以MPR影像,对比分析A、B、C 3组对病变定位及定性诊断与手术病理诊断符合率。结果:A组确定病变部位13例(46.43%)、病变性质14例(50%),B组确定病变部位18例(64.28%)、病变性质17例(60.71%),C组确定病变部位23例(82.14%)及病变性质23例(82.14%)。A、B、C 3组对病变定位及定性诊断差异有统计学意义(P<0.05),A、B两组对病变定位及定性的诊断无统计学意义(P>0.05),而C组对病变定位和定性诊断方面均优于A或/和B组,A组与C组比较有统计学意义(P<0.05)。结论:对于腹腔巨大囊性病变,多层螺旋CT横断面辅以MPR技术可较准确的显示病变的部位、形态、范围及周围结构关系,对临床诊断和治疗有重要价值。Objective: Explore and discuss the value of multi-slice spiral CT (MSCT) multiple planar reconstruction technique in di- agnosing the abdominal giant cystic lesions. Methods: 28 cases of CT findings of giant cystic lesions in abdomen proved by pathology were retrospectively analyzed ,which were classified into group A (axial spiral CT images) , group B (MPR post-processing images) and group C (axial spiral CT images with MPR post-processing images) ,and their lesion localization, qualitative diagnosis and surgical pa- thology were comparatively analyzed. Results: In group A,the lesion locations of 15 cases (53.57%) were confirmed,and 14 cases (50%) were qualitatively confirmed,and that of group B were 18 cases (64.28%) and 17 cases (60.71%). In group C the lesion locations of 23 cases (82.14%) were confirmed,and 23 cases (82.14%) were qualitatively confirmed. Group C is superior to group A or/and B. There was no significant difference between group A and group B (P 〉 0. 05 ). The differences between group A and group C were statistically valid (P 〈 0. 05). Couclusion:MSCT multiple planar reconstruction technique has high clinical value in di- agnosis and treatment of abdominal giant cystic lesion. It is because it can provide the information of position, shape and range of peri- toneal cystic lesions.
