摘 要:目的:近年MicroRNAs作为诊断急性心肌梗死的一类新生物标记物成为研究的热点。本研究对MicroRNA-126(miR-126)在急性心肌梗死急性期的表达进行了检测。方法:本研究纳入急性心肌梗死(acute myocardial infarction,AMI)患者36例,正常对照组患者34例。在心肌梗死症状发作后的不同时间段:AMI患者入院时(T0),AMI发病8 h(T8),AMI发病12 h(T12),AMI发病24 h(T24),AMI后第7天采集血液样本并应用实时定量PCR法对全血中miR-126表达水平进行检测。应用酶联免疫吸附测定(enzyme-linked immunosorbent assay,ELISA)法检测血浆肌钙蛋白I(plasma troponin I,cTnI)的表达水平。结果:急性心肌梗死患者全血中miR-126的表达水平在T0、T8、T12、T24时间点均较对照组明显升高且有统计学意义(P<0.05);而AMI后第7天时其表达水平与对照组无明显差异且无统计学意义(P>0.05)。AMI早期全血miR-126与cTnI的显示了相同的升高趋势。受试者工作特征曲线(ROC曲线)分析结果显示全血miR-126在T0、T8、T12、T24时间点的ROC曲线下面积(area under curve,AUC)分别为0.857,0.863,0.827,0.789(P值均<0.05)。结论:在心肌梗死早期miR-126在AMI早期表达水平明显升高,且与cTnI相同的时间窗,但拥有更早的达峰时间。这提示检测全血miR-126可能作为早期诊断急性心肌梗死的有效方法。Objective:In recent years,as a new biological marker in the diagnosis of acute myocardial infarction MicroRNAs become a hot research. The study investigates the expression of MicroRNA-126 (miR-126) in the acute phase of acute myocardial infarction. Methods: The study included 36 patients in the group of acute myocardial infarction (AMI) ,while 34 patients in the normal control group. In the different periods: the time of AMI patients with admission to hospital (TO) ,AMI incidence 8 h (T8) , AMI incidence 12 h (T12) ,AMI incidence 24 h (T24) ,AMI 7 days after the collection of blood samples and apply real-time quantitative PCR method to detect the expression level of miR-126 in whole blood. Enzyme-linked immunosorbent assay (ELISA) is used for detecting plasma tro- ponin I (cTnI) expression levels. Results: The expression levels of whole blood miR-126 of patients with acute myocardial infarction at T0,T8, TI 2,T24 time points are significantly higher than those of the control group and was statistically significant (P 〈 0.05 ) ;the expression level after 7days shows no difference with the control group and there is no statistical significance (P 〉 0.05). In the early stages of AMI, MiR-126 and eTnI in whole blood show the same increasing trend. The receiver operating characteristic curve (ROC) shows that the area under curve (AUC) of whole blood miR-126 at the T0,T8 ,T12,T24 time points were 0. 857,0. 863,0. 827 ,and 0. 789 (P values 〈 0.05 ). Conclusion: In the early stage of myocardial infarction AMI,the expression level miR-126 was significantly higher,with the same time window but earlier peak time with cTnI. It is suggested that the whole blood miR-126 may be used as an el-fective method for early diagnosis of acute myocardial infarction.
