乳腺肿块微创旋切术的临床应用分析

摘　　要：目的：探讨超声引导下乳腺肿块微创旋切术的应用及技术改进。方法：回顾性分析利用EnCor真空辅助旋切系统对323例乳腺肿物患者进行手术治疗与随访的资料。结果：全部乳腺肿物均顺利切除。术后病理：纤维腺瘤233例，乳腺增生153例，不典型增生5例，导管扩张13例，乳头状瘤2例，乳腺腺病8例，炎性肿块12例，乳腺癌1例。其中有92例有两个及以上诊断。所有病理结果阳性率100％。术后并发症：皮下瘀斑16例，血肿形成3例。289例随访3～12个月，平均6个月，均未发现病灶残留、复发、且无切口瘢痕形成，乳房外形正常，皮肤触觉无异常。结论：超声引导下乳腺肿块微创旋切术作为治疗乳腺肿物的方式具有切除准确、安全有效、美容、并发症少等优点。可对乳腺良性病灶进行切除，也可用于乳腺恶性肿瘤手术前活检。Objective： To explore the implications and improvements of breast tumor excised by ultrasound guided vacuum-assisted breast biopsy. Methods：Retrospective analysis of 323 breast tumor patients by Encor vacuum-as- sisted breast biopsy. Results：All breast tumors were removed successfully. The postoperative pathology：233 cases of fi- broadenoma, 153 cases of breast hyperplasia, 5 cases of atypical hyperplasia, 13 cases of catheter dilatation,2 cases of pa pillary tumor, 8 cases of breast adenosis, 12 cases of inflammatory mass, 1 case of breast cancer. There were 92 cases with two and more diagnosis. The positive rate of all pathological results was 100%. Postoperative complications： 16 cases of subcutaneous ecchymosis,3 cases of hematoma. 289 cases were followed up, ranging from 3 to 12 months, average 6 months. Recurrence,residue and scar formation were not found. And the shape of breast and skin tactile were normal. Conclusion： Ultrasound-guided vacuum-assisted breast biopsy is accurate, safe and effective, of cosmetology and fewer complications. The benign breast neoplasms are resected by ultrasound-guided vacuum-assisted breast biopsy and it can al- so be used for malignant breast tumor biopsy before the surgery date.
