玻璃酸钠注射液对膝关节骨性关节炎疗效的观察性研究

摘　　要：目的探讨玻璃酸钠对口服药物效果欠佳的单纯膝关节骨性关节炎的治疗效果。方法选取诊断明确的,希望继续门诊保守治疗,且经过至少一种消炎镇痛药治疗无效或效果欠佳的膝关节骨性关节炎患者125例,不经过药物洗脱期,直接改用玻璃酸钠注射液进行关节腔内注射,每周1次,共5次,比较治疗前后ISOA指数7项指标及VAS评分的差异性,评估玻璃酸钠对骨性关节炎的疗效。结果经过一个共5次关节腔内注射的标准治疗后,所有患者的病情均得到不同程度的缓解(ISOA7项指标总分治疗前后比较P<0.05),其中对走路时疼痛及最大步行距离改善最为明显(P<0.001)。结论玻璃酸钠关节腔内注射对未经药物洗脱的患者依然能收到明显的效果,对走路时疼痛及最大步行距离改善最为明显。Obejective To investigate the effects of sodium hyaluronate in treatment of knee arthritis which has poor efficacy of oral medications. Methods Totallly 125 patients who required outpatient treatment were enrolled in this study. All the patients were diagnosed as knee osteoarthritis only, and had been treated by at least one sort of anti - inflammatory analgesics with poor efficacy. There was no washout period for all the patients. Sodium hy- aluronate injection was directly injected into the joint cavity once a week for five weeks. ISOA index and VAS score were evaluated to assess the therapeutic effects. Results After the five- time injection procedure, all the patients were improved remarkably（ISOA seven index scores compared before and after treatment P〈0.05）, espatially the walking- pain relief and maximum walking distance（P〈0. 001）. Conclusions Intraarticular injection of sodium hyaluronate can effectively improve the maximum walking distance and relieve the walking - pain of patiens who have poor efficacy with oral medication.
