NICU常规机械通气相关并发症临床分析

摘　　要：目的总结NICU常规机械通气的相关并发症,分析其原因。方法回顾近年本院NICU常规机械通气治疗中出现相关并发症的患者36例,总结其临床变化、实验室或辅助检查、机械通气指标和医护处理。结果并发症包括呼吸机相关性肺炎19例、插管过深15例、脱管5例、通气过度9例、肺气压伤9例、咽喉或气管上段损伤12例、早产儿视网膜病1例;主要与医疗措施或护理操作不当、医院感染、患儿本身危重等因素有关。结论临床医生应提高机械通气的理论和操作水平,遵守无菌原则,密切监测病情,以减少医源性并发症。Objective To sum up the adverse effects of routinal mechanical ventilation in NICU and analyze their causes. Methods Totally 36 cases of NICU were reviewed in author＇s hospital with adverse effects of routinal mechianical ventilatory therapy from Oct 2009 to Oct 2012. Their clinical courses, laboratory or accessory examinations and clinical or nursing procedures were sumed up. Results Adverse effects, including pulmonary infection or worsening（19 cases）, deep encheiresis（15 cases）, out of intubation（5 cases）, hyperventilation（9 cases）, airway trauma（9 cases）, pharyngolarygeal or upper trachea trauma（12 cases） and retinopathy of prematurity（1 cases） , were mainly related to factors such as inadequate clinical or nursing procedures, cross infection in hospital, bad condition of patient himself, and so on. Conclusion In order to reduce the iatrogenic adverse effects, we should improve the knowledge and technique of mechanical ventilation, and observe the aseptic rules and closely monitor the patient＇s condition.
