摘　　要：目的探讨应用三维外固定支架结合有限内固定治疗胫骨中下段粉碎性骨折的临床效果。方法回顾性分析2007年3月-2011年3月期间应用三维外固定支架结合有限内固定治疗胫骨中下段粉碎性骨折28例患者，其中开放性骨折6例，闭合性骨折22例，根据AO分类，A型6例，B型8例，c型14例，并于术后4-6个月拆除外固定支架，术后随访6-24个月（平均16个月）。结果26例患者骨折顺利愈合（92．8％），愈合时间为3-6个月，平均为3-8个月，2例患者出现骨折延迟愈合，均为开放性骨折，合并有软组织感染。结论应用三维外固定支架结合有限内固定治疗胫骨中下段粉碎性骨折具有操作简便、并发症少、稳定性强、骨折愈合快、拆除简单、不需要二次手术等优点。Objective Discuss the clinical effect of treatment of middle-distal tibial comminuted fractures with three di mensional external fixation and limited internal fixation. Methods From March 2007 to March 2011,28 patients with a middle-distal fracture were treated with the three dimensional external fixation and limited internal fixation and removed the external fixation 4 to 6 months later after operation. 6 patients were open fracture. All the patients were followed up from 6 months to 24 months （mean 16 months）. Results 26 fractures （92.8%） healed tun eventfully at a mean of 3.8 months （range 3 to 6 months）. But 2 cases were delayed-unions associated with open fracture and infection （7.1%）. Conclusion The application of three-dimensional external fixator combined with limited internal fixation in the treatment of middle-distal tibial comminuted fractures has many advantages such as simple operation, lower incidence rate of complicating diseases, better stability, shorten time of fractures healing, removed simply, and do not need twice operation. So it is suitable for middle and distal tihial comminuted fractures, especially for open fracture.
