贝前列素钠对提高老年患者动静脉内瘘术成功率的观察

摘　　要：目的观察贝前列素钠对老年患者动静脉内瘘的作用与效果。方法本研究纳入82例老年患者行动静脉内瘘术患者,随机分为贝前列素钠治疗组42例,对照组40例,对2组动静脉内瘘术成功率、并发症发生率、内瘘术后血管及血流变化情况、血常规及凝血分析变化、药物不良反应进行临床观察和比较。结果治疗组动静脉内瘘术后2周及4周成功率(97.6%、97.6%)比对照组高,差异有统计学意义(χ2=12.48,P=0.002;χ2=11.72,P=0.014),治疗组术后并发症发生率(4.8%)比对照组术后并发症发生率(55%)低,差异有统计学意义(χ2=21.45,P=0.012),内瘘术后四周血流量(406.4±108.4)ml/min与对照组术后四周血流量(228.20±88.6)ml/min比较,差异有统计学意义(F=246.238,P=0.000),治疗组术前及术后血常规及凝血分析比较,差异无统计学意义(P>0.05)。结论贝前列素钠能够提高老年患者动静脉内瘘术的成功率,且疗效确切。Objective To study beraprost sodium therapy on the success rate of arteriovenous fistula in elderly patients.Methods A total of 82 elderly patients subjected to venous fistula surgery were enrolled in this study,and were randomly assigned into beraprost sodium therapy group（treatment group,n=42） or control group（n=40）.After the arteriovenous fistula surgery,the success rate of arteriovenous fistula,prevalence of complications,changes of blood vessels and blood flow,routine blood analyses,changes of blood coagulation,and adverse drug reactions were compared between the two groups.Results In treatment group,the success rate of arteriovenous fistula after the surgery for 2 and 4 weeks was 97.6%,significantly higher than that in the control group（after 2 weeks,χ2=12.48,P=0.002;after 4 weeks,χ2=11.72,P=0.014）.The prevalence of postoperative complications was 4.8% in treatment group and was 55% in control group（χ2=21.45,P=0.012）.Blood flow in fistula after the surgery for 4 weeks was 406.4 ± 106.4ml/min in treatment group,and was 228.20±88.6ml/min in control group（F=246.238,P =0.000）.Routine blood analyses and blood coagulation changes showed no differences between the two groups（P0.05）.Conclusion Beraprost sodium can increase the success rate of internal arteriovenous fistula operation in elderly patients.
