摘  要:目的探讨呼吸机管道处理方法对慢性阻塞性肺疾病(COPD)患者呼吸机相关性肺炎(VAP)的影响。方法慢性阻塞性肺疾病患者60例根据随机对照的方法分为观察组30例与对照组30例,两组都采用机械通气治疗,对照组每天更换呼吸机管道,观察组每5天更换呼吸机管道。结果观察组的VAP发生率为10.0%,对照组VAP发生率为40.0%,对比差异有统计学意义(P<0.05)。两组治疗前APACHE-Ⅱ评分对比无明显差异,治疗后都有明显下降(P<0.05),同时组间对比差异有统计学意义(P<0.05)。年龄、通气时间、呼吸机管道更换时间与APACHE-Ⅱ评分为主要的危险因素(P<0.05)。结论合理进行呼吸机管道处理,有利于降低COPD患者呼吸机相关性肺炎发生率,提高预后效果,同时要根据危险因素加强预防与管理。Objective To investigate the influence of ventilator tube treatment on ventilator-associated pneumonia(VAP)in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 60cases with COPD were equally divided into observation group and control group.All cases were treated with mechanical ventilation.Patients in control group were replaced the ventilator tube daily,and those in observation group was changed every 5days.Results The incidence of VAP of observation group was 10.0%,lower than the 40.0%in control group(P0.05).APACHE-Ⅱscore of the two groups before treatment had no significant difference,both of which were decreased significantly after treatment(P0.05).Age,APACHE-Ⅱscore,duration of ventilation and the ventilator tube replacement time were the major risk factors(P0.05).Conclusion Rational ventilator pipeline processing could help to reduce the VAP incidence in COPD patients and improve the prognosis.
