脂肪细胞因子及炎性反应与代谢综合征患者胰岛素抵抗的相关性研究

摘　　要：目的通过观察代谢综合征(MS)患者血清脂联素(APN)、内脂素(Visfatin)、高敏C反应蛋白(hsCRP)水平及胰岛素抵抗指数(HOMA-IR)值的变化与相关性,探讨脂肪细胞因子及炎性反应与MS的IR相关性。方法检测117例MS患者和50例对照组的血清APN、Visfatin、hs-CRP水平及HOMA-IR值,进行统计学分析。结果 MS患者除HDL-C略低于对照组(P>0.05)外,其他指标均高于对照组(P>0.01)。MS患者血清Visfatin及hs-CRP水平与HOMA-IR值呈显著正相关(r=0.727、0.801,P<0.01),血清APN水平与HOMA-IR值呈显著负相关(r=-0.690,P<0.01),血清APN水平与Visfatin水平呈显著负相关(r=-0.853,P<0.01),其相关性良好。结论 MS患者体内存在血糖和血脂的代谢紊乱、脂肪细胞因子水平异常表达、炎性反应及IR,且脂肪细胞因子及炎性反应与IR具有相关性。Objective To analyze the correlation between fat cells factors,inflammatory reation and insulin resistance（IR）in patients with metabolic syndrome（MS）.Methods Serum levels of adiponectin（APN）,visfatin,high sensitivity C reactive protein（hs-CRP）were detected and HOMA IR index（HOMA-IR）value was calculated in 117cases of MS patients and 50healthy subjects,and the results were statistically analyzed.Results Levels of most indicators in patients with MS,excepting HDL-C（P0.05）,were higher than control group（P0.01）.In MS patients,serum levels of visfatin and hs-CRP were positively correlated with HOMA-IR value（r=0.727or 0.801,P0.01）,serum APN level was negatively correlated with HOMA-IR value（r=-0.690,P0.01）and serum APN level was negatively correlated with visfatin level（r=-0.853,P0.01）.Conclusion There could be blood glucose and lipids metabolism disorders,abnormal expression of fat cell factors,inflammatory reaction and IR in patients with MD,and there might be correlation between fat cell factors,inflammatory reaction and IR.
