摘 要:目的观察蛇形管内窥镜在双腔支气管插管定位中应用的效果。方法选择本院拟行双腔气管插管患者60例,采用随机双盲法分为蛇形管内窥镜组(A组)和听诊组(B组),每组30例。插管成功后A组使用蛇形管内窥镜插入双腔导管的主管内观察导管前端位置,调整导管前端到合适的位置并妥善固定;B组单纯使用听诊法听诊双肺呼吸音调节导管深度。比较两组用时和调整后单肺通气效果。结果两组患者调整导管时间比较,A组用时明显短于B组,并且开胸后肺叶萎陷良好例数A组也较B组多(P<0.05)。结论蛇形管内窥镜可以方便快捷地用于双腔气管插管的定位,其定位准确性优于单纯使用听诊法。Objective To observe the application effect of serpentuator endoscope in the location of double-lumen tube intubation. Methods Selected 60 patients who planned to receive the double-lumen tube intubation were randomized and double blind divided into serpentuator endoscope group (group A) and auscultation group (group B), 30 cases in each group. After successful intubation, serpentuator endoscope were inserted into the chord member of double-lumen tube to observe the front position of the catheter for patients in group A, then adjusted the front position of the catheter to appropriate location and properly fixed the catheter; patients in group B were treated with stethoscopy alone to auscultate lung respiratory sound and then adjust the depth of catheter. The time and the effect of one-lung ventilation were compared. Results The time of adjusting catheter of group A was significantly shorter than that of group B, and the good number of pulmonary collapse after tboracotomy were more than group B (P 〈 0.05). Conclusion Serpentuator endoscope is quick and convenient to locate double-lumen endotracheal intubation, and its positioning accuracy is better than that of pure stethoseopy,
