甲亢患者术前术后不口服碘剂的临床研究

摘　　要：目的探讨甲状腺功能亢进症(甲亢)患者术前术后不口服碘剂的可行性。方法将56例甲亢患者按是否口服碘剂随机分为实验组(术前术后不口服碘剂组)29例和对照组(术前术后口服碘剂组)27例,比较两组间手术时间、出血量、术后FT3、术后FT4、住院时间和并发症发生率的差异。结果实验组的手术时间、出血量、术后FT3、术后FT4及住院时间分别为(96.32±8.38)min、(74.21±6.32)mL、(3.96±1.02)pmol/L、(9.88±1.86)pmol/L、(9.58±0.34)d,对照组的手术时间、出血量、术后FT3、术后FT4及住院时间分别为(85.63±3.97)min、(70.00±7.18)mL、(4.08±0.93)pmol/L、(9.68±1.74)pmol/L、(28.13±1.48)d,两组间手术时间、出血量、术后FT3、术后FT4均无显著性差异(P>0.05),二组间住院时间有非常显著性差异(P<0.01),两组患者均未发生甲状腺危象。结论甲亢患者术前术后不口服碘剂是安全可行的。Objective To study the feasibility of hyperthyroidism patients before and after surgery without iodine ad- ministration. Methods All patients according to whether oral iodine were randomLy divided into experimental group and control group, were compared the operation time,bleeding volume,postoperative FF3,postoperative FT4,hospital- ization time and complication rate. Results The operated time,the bleeding volume,postoperative FT3,postoperative FT4 and hospitalized time in experimental group were （96.32±8.38） min, （74.21±6.32） ml, （3.96±1.02） pmol/L, （9.88± 1.86） pmol/L and （9.58±0.34） d, The operated time, the bleeding volume, postoperative FT3,postoperative FT4 and hos pitalizatized time in control group were （85.63±3.97） min, （70±7.18） mL, （4.08±0.93） pmol/L, （9.68±1.74） pmol/L and （28.13±1.48） d, there was no significant difference between the two groups of operated time,bleeding volume,post operative FT3 and postoperative FT4 （P〉0.05）, there was a significant difference between the two groups hospitalized time （P 〈 0.01 ）, all patients had no thyroid crisis. Conclusion In patients with hyperthyroidism before and after operation without oral iodine agent is safe and feasible.
