应用牛颈静脉行上腔静脉置换治疗上腔静脉阻塞综合征的临床观察

摘　　要：目的探讨应用牛颈静脉行上腔静脉置换治疗上腔静脉阻塞综合征的可行性。方法 2008年2月—2011年5月收治胸部肿瘤侵犯上腔静脉引起上腔静脉阻塞综合征12例,均应用牛颈静脉置换受肿瘤侵犯的上腔静脉。结果本组术后均恢复顺利,术后12～36 h上腔静脉阻塞症状缓解。术后所有患者接受化疗,2例恶性胸腺瘤术后6个月接受局部放疗。牛颈静脉1年通畅率为100%。1例肺癌术后18个月肿瘤复发未接受治疗,1例肺癌和2例恶性胸腺瘤均于术后24个月死亡。随访期间未见移植的牛颈静脉内血栓形成及钙化。结论应用戊二醛处理的牛颈静脉对胸部肿瘤引起的上腔静脉阻塞综合征行上腔静脉置换治疗安全可行。Objective To investigate the feasibility of replacement of superior vena cava by using bovine jugular veins in treatment of superior vena eaval obstruction syndrome. Methods Twelve patients with superior vena eaval ob- struction syndrome induced by thoracic neoplasms encroachment admitted during February 2008 and May 2011 underwent the replacement of encroachment of superior vena cava by using bovine jugular veins. Results Operation treatments suc- cessfully recuperated all patients, and symptoms of superior vena cava were relieved postoperative 12 -36 h. All patients underwent postoperative chemotherapy, and 2 patients with malignant thymoma underwent part radiotherapy 6 months af- ter operation. One year of patency rate of bovine jugular veins was 100%. One patient with pulmonary cancer relapsed 18 months after operation without treatment, one patient with puhnonary cancer and 2 patients with malignant thymoma died 24 months after operation. There was no thrombus or calcification in transplanted bovine jugular veins during the follow- up period. Conclusion Replacement of superior vena eava by using bovine jugular vein treated with glutaral in treatment of superior vena eaval obstruction syndrome is feasible and safe.
