中风后痉挛性瘫痪中医护理方案的临床应用

摘　　要：目的观察综合性中医护理方案对中风后痉挛性瘫痪的影响,为研究有效的护理方案提供临床研究依据。方法将100例中风后痉挛性瘫痪住院病人随机分为试验组和对照组各50例,分别采用综合性中医护理方案和常规护理方法进行护理后,运用Brunnstrom评定瘫痪状态等级变化、改良Ashworth法评定痉挛状态、改良Barthel指数(MBI)评分量表评估日常生活能力。结果对照组MBI评分护理前后差异有统计学意义(P<0.05);试验组Brunnstrom分级、Ashworth痉挛分级、MBI评分护理前后差异均有统计学意义(P<0.05);且护理后MBI评分试验组优于对照组(P<0.05)。结论综合性中医护理方案可明显改善中风后痉挛性瘫痪患者肢体的痉挛程度,是一套有效的护理方案。Objective To observe the clinical effects of TCM nursing program on spastic paralysis after stroke and provide clinical research basis for an effective nursing plan. Methods 100 patients with spastic paralysis after stroke were randomly divided into experimental group and control group with 50 cases in each group. The experimental group was given TCM nursing while control group was given routine nursing. The clinical effects were evaluated by the paralyzed state level with Brunnstrom classification assessment, the spasticity state with modified Ash- worth method and the daily life ability with modify Barthel index （MBI） Rating Scale. Results In control group Barthel index （MBI） score had statistically significant difference before and after treatment（P〈0.05）; in experimental group Brunnstrom classification, Ashworth spasticity, Barthel index （MBI） score had statistically significant differences before and after treatment （P〈0.05）; and the Barthel index（MBI） score of experimental group was better than that of control group after care（P〈0.05）. Conclusion TCM nursing program can significantly improve the limb spasticity for patients with spastic paralysis after stroke therefore it is an effective care method.
