针灸推拿综合康复疗法改善脑卒中患者运动功能临床研究

摘　　要：目的观察针灸推拿综合康复疗法改善脑卒中缓解期患者运动功能的临床疗效。方法将40例脑卒中缓解期病例随机分为治疗组和对照组,每组20例。两组均予包括肢体摆放、坐位平衡训练、患肢持重训练、步行训练、上下楼梯训练、日常生活活动训练、痉挛肌电刺激等临床常规康复措施,治疗组在此基础上加用针灸、推拿疗法。两组疗程均为4周,分别于治疗前、治疗2周、治疗4周观察Fugl-Meyer运动功能评定量表(FMA)和功能独立性评定量表(FIM)积分变化情况。结果两组治疗2周与治疗前比较,以及治疗4周与治疗2周比较,FMA、FIM积分均有提高(P<0.05);治疗组治疗2周、治疗4周FMA积分明显高于对照组(P<0.05),第4周FIM积分高于对照组(P<0.05)。结论针灸推拿综合康复疗法对脑卒中运动功能障碍的恢复有较持续稳定的促进作用。Objective To observe the clinical efficacy of acupuncture and massage on improvement of motor function in stroke patients. Methods Fourty cases were randomized into treatment group and control group, 20 cases in each group. All the patients in the two groups were treated with routlue rehabilitation therapy, and patients in treatment group were treated with acupuncture and massage additionally, with the course of 4 weeks. The FMA score and FIM score were observed before treatment, after 2 weeks treatment and 4 weeks treatment. Results After 2 weeks treatment, the FMA score and FIM score were higher than before treatment, and after 4 weeks treatment, the FMA score and FIM score were higher than after 2 weeks treatment（ P 〈 0.05 ）. After 2 or 4 weeks treatment, the FMA score in treatment group was higher than that of the controls （P 〈 0.05 ）. After 4 weeks treatment, the FIM score was higher than that of the controls （P 〈 0.05）. Conclusion Comprehensive rehabilitation therapy of acupuncture and massage can promote the recovery of motor dysfunction in stroke oatients.
