337株肠杆菌科细菌超广谱β-内酰胺酶的检测及耐药分析

摘  要:目的 :测定337株肠杆菌科细菌产超广谱β-内酰胺酶 (ESBLs)情况 ,了解产酶株组与非产酶株组对临床常用抗生素的耐药情况。方法 :用双纸片协同试验筛选产ESBLs株 ,采用K -B法测定产酶株与非产酶株对临床常用抗生素的敏感性。结果 :31株肠杆菌科细菌为产ESBLs株。1998~1999年度产酶率 (12 9 % )显著高于1996~1997年度产酶率 (4 2 % )。产酶株组对3代头孢和氨曲南、安美汀、特美汀耐药率显著高于非产酶株组 ,经x2 检验 ,P<0.05(0 01)。但对亚胺培南、环丙沙星、丁胺卡那耐药率较低。结论 :对肠杆菌科细菌感染应注意ESBLs的检测。临床应慎用3代头孢菌素。对产ESBLs株可选用亚胺培南、环丙沙星、丁胺卡那霉素治疗。Objective To detect the extended-spectrum β-lactamases(ESBLs)and resistance rate to usually used antibiotics in members of the family Enterobacteriaceae. Methods:Double disk test and K-B test was performed to detect ESBLs and antibiotic resistance rate. Results:31 ESBL-producing isolates was assessed in 337 clinical isolates;ESBLs-producing isolates usually was mutiple antibiotic resistance, the resistance rate is obviously increasing than non-ESBLs-producing organisms to third-generation cephalosporins, aztreonam augmentin and timentin. Conclusions:It may be clinical necessary to institute additional testing on a routine basis to detect ESBLs production in all clinical isolates of the family Enterobacteriaceae, and the third-generation cephalosporins should be used cautiusly in clinic.
