强力圈、椎板下钢丝和椎弓钉联合治疗腰椎滑脱

摘　　要：总结强力圈、椎板下钢丝和椎弓钉联合治疗腰椎滑脱的优越性。方法　应用自行设计的强力圈和滑脱复位椎弓钉加椎板下钢丝治疗腰椎滑脱 32例。滑脱程度Ⅰ度 19例 ,Ⅱ度 9例 ,Ⅲ度 4例 ;采用复位器进行复位、后路椎间和关节突及横突周围植骨。结果　术后随访 1年以上 ,解剖复位率 94 5 % ,植骨全部融合。结论　本方法的使用避免了单独使用椎弓钉复位引起的术中椎弓钉松动滑脱 ;减少了椎弓钉的使用数量 ;克服了椎弓钉进钉点受器械限制的缺点 。Objective To study the effectiveness of the surgical treatment of spondylolisthesis with the combined use of a self designed device consisting of the reinforced ring under the lamina,pedical nail and wiring fixation for the reduction.Methods 32 cases of spondylolisthesis were treated with this method.The lesions were of the Ⅰ degree in 19 cases,Ⅱ degree in 9 cases and Ⅲ degree in 4 cases.The spondylolisthesis were reduced with the reduction apparatus.Through the posterior approach,bone graft was done around the facets and the transverse processes.Results The follow up period extended for more than one year.Anatomical reduction was attained in 95.4% of the cases with healing of the bone graft in all.The functional recovery was excellent in 88.9%.Conclusion The application of this method avoided intra operative loosening of the pedicle nails;the number of pedicle nails used could be reduced;the sites of nail insertion were selected without the restrictions by instrumentation.The operation period was shortened as well.
