摘  要:目的:探讨宁夏回、汉族人群脂联素(APN)基因+45位核苷酸T/G多态性,及其血清水平与冠心病(CHD)的相关性。方法:收集回族CHD患者95例(CHD组)、正常对照98例(对照组),和汉族CHD患者93例(CHD组)、正常对照102例(对照组)。采用聚合酶链反应-限制性片段长度(PCR-RFLP)多态性技术和酶联免疫吸附法(ELISA),对各组APN基因+45位核苷酸T/G多态性及其血清APN浓度进行检测。结果:回、汉族CHD组间及对照组间基因型频率及等位基因频率均差异有统计学意义(均P<0.05)。回族CHD组TG+GG基因型频率及G等位基因频率均高于回族对照组(均P<0.05);汉族CHD组TT基因型频率及T等位基因频率均高于汉族对照组(均P<0.05)。回、汉族CHD组血清APN水平均低于对照组(均P<0.05);回族TG+GG基因型血清APN水平低于TT基因型者,汉族TT基因型血清APN水平低于TG+GG基因型者。Logistic回归分析显示,回族携带TG+GG基因型者发生CHD的风险增加(OR=46.393,95%CI:1.486~144.790)。结论:APN基因+45T/G多态性在回、汉族人群之间存在差异;回、汉族CHD患者血清APN均低于正常人群;APN基因+45T/G多态性与宁夏回、汉族人群CHD发病均具有相关性。Objective:To investigate relationships between adiponectin (APN) gene +45 nucleotide T/G poly- morphism, serum APN level and coronary heart disease (CHD) in Hui and Han population in Ningxia Hui auton- omous region. Methods:95 Hui patients and 93 Han patients with CHD (CHD group) as well as 98 Hui healthy people and 102 Han healthy people (control group) were included in the present study. Polymerase chain reaction- restriction fragment length polymorphism (PCR-RFLP) and enzyme-linked immunosorbent assay (ELISA) were used to detect the gene polymorphism of APN and the serum APN level respectively. Results:The frequencies of genotype and allele were significantly different between Hui CHD group and Han CHD group (both P〈0.05), there were also significant differences between Hui control group and Han control group. The frequencies of TGq- GG genotype and G allele in Hui CHD group were higher than those in Hui control group (both P〈0.05). The frequencies of TT genotype and T allele in Han CHD group were higher than those in Han control group (both P〈0.05). Compared with control group, the serum APN level in CHD group was lower in both Hui and Han population (both P〈0.05). The serum APN level in subjects with TG+GG genotype was lower than that with TT genotype in Hui population (P〈0.05), the serum APN level in subjects with TT genotype was lower than that with TG+GG genotype in Han population (both P〈0.05). Logistic regression analysis showed that individ- uals carrying TG+GG genotype had an increased risk of CHD in Hui population (OR=46. 393, 95%CI: 1. 486-- 144. 790). Conclusion:There is racial difference between H ui and H an population with APN +45 gene polymor- phism. The serum APN level in CHD patients is reduced in both Hui and Han population. The APN +45 T/G polymorphism might be associated with CHD in Hui and Han population in Ningxia Hui autonomous region.
