瑞舒伐他汀联合阿司匹林对颈动脉粥样硬化患者血脂、血清炎症因子及颈动脉内-中膜厚度的影响

摘　　要：目的探讨瑞舒伐他汀联合阿司匹林对颈动脉粥样硬化(CAS)患者血脂、血清炎症因子及颈动脉内-中膜厚度(IMT)的影响。方法将220例CAS患者随机分为瑞舒伐他汀组(n=72)、阿司匹林组(n=73)和联合治疗组(n=75),分别给予口服瑞舒伐他汀10 mg、阿司匹林100 mg或瑞舒伐他汀10 mg联合阿司匹林100 mg,均每日1次,持续6个月。观察治疗前后血脂水平、炎症因子以及IMT和粥样斑块数量的变化。结果治疗6个月后,联合治疗组和瑞舒伐他汀组患者血脂水平均显著改善(P<0.05),两组间无显著差异(P>0.05);而阿司匹林组治疗前后血脂水平无显著改变(P>0.05)。联合治疗组各炎症因子水平改善情况优于其他两组(P<0.05);三组的颈动脉IMT及斑块数量均显著减小(P<0.01);联合治疗组颈动脉IMT及斑块数量显著小于其他两组(P<0.05或P<0.01)。结论瑞舒伐他汀联合阿司匹林可调节血脂、抑制血栓形成、抗炎、抗氧化和逆转粥样斑块。AIM To explore the effects of rosuvastatin and aspirin in combination on blood lipid, inflammatory factor and carotid intima-media thickness （IMT） in patients with carotid atherosclerosis （CAS）. METHODS Two hundreds and twenty cases of CAS patients were randomly divided into rosuvastatin group （rosuvastatin 10 rag, n = 72）, aspirin group （aspirin 100 mg, n = 73） and combined group （rosuvastatin 10 mg ＋ aspirin 100 mg, n = 75） . The levels of blood lipid, inflammatory factors, IMT and atheromatous plaque number were examined. RESULTS After six months treatment, the blood lipid levels were significantly decreased in combined group and rosuvastatin group （P 〈 0.05）, without significant difference between thesetwo groups （P 〉 0.05） . However, aspirin treatment didn＇ t alter the levels of blood lipid （P 〉 0.05） . The expressions of inflammatory factors in combined group were significantly reduced than those in the other two groups （P 〈 0.05）. The values of IMT and atheromatous plaque were decreased remarkably in all three groups （P 〈 0.01） and combined group showed the greatest reduction （P 〈 0.05 or P 〈 0.01）. CONCLUSION The combination of rosuvastatin and aspirin shows better effects on adjusting blood lipid, restraining thrombosis, anti-inflammation, anti-oxidation and reversing atheromatous plaque formation.
