摘 要:目的探讨老年人血清25-羟基维生素D3[25(OH)D3]与MS的相关性。方法选取MS患者65例及健康者62名,测量血压、身高、体重、WC及臀围,检测25(OH)D3、FPG、FIns、TC、TG、HDL-C、LDL-C及血尿酸(SUA),计算WHR、BMI及胰岛素抵抗指数(HOMA-IR)。结果 MS组25(OH)D3水平低于NC组(P<0.05)。按照25(OH)D3≥30ng/ml、20ng/ml≤25(OH)D3<30ng/ml、25(OH)D3<20ng/ml水平分为3组,NC组在各组中所占比例分别为30.6%、27.4%、41.9%;MS组在各组中所占比例分别为4.6%、33.8%、61.6%。MS组25(OH)D3缺乏比例高于NC组(P=0.012)。MS组25(OH)D3水平与BMI、WC、SBP、FPG、HOMA-IR、TG、LDL-C、SUA呈负相关(P<0.05),与WHR、DBP、FIns、TC呈负性线性趋势(P>0.05),与HDL-C呈正性线性趋势(P>0.05)。根据BMI不同水平将MS组分为超重和肥胖亚组,两亚组25(OH)D3水平比较差异无统计学意义。结论正常老年人维生素D水平普遍较低,MS老年患者更低。MS各组分与25(OH)D3水平呈负相关,提示25(OH)D3低水平可能是MS的危险因素。Objective To investigate the relationship between serum 25-hydroxy-viamin D3[25(OH)D3 ] concentration and MS. Methods The 65 outpatients with MS and the 62 healthy subjects were recruited in the study. All participants were given a physical examination, including determination of weight, height, body weight, WC, hip circumference, and blood pressure. After an over night fast, the peripheral venous blood samples were collected for measuring the concentrations of 25(OH)D3, FPG, Fins, TC, TG, HDL-C, LDL-C, and serum uric acid (SUA). WHR, BMI, and HOMA-IR were calculated for analysis. Results The mean concentration of 25(OH)D3 was lower in the MS group than in the NC group (P〈0. 05). The 25 (OH) D3 sufficiency, insufficiency and deficiency were 30.6%, 27.4%, and 41.9% respectively in the NC group, and 4.6%, 33.8%, and 61.6%respectively in the MS group. The percentage of 25 (OH)D3 deficiency in the MS group was significantly more prevalent than in the NC group (P= 0. 012). In the MS group, the concentration of 25 (OH)D3 correlated negatively with BMI, WC, SBP, FPG, HOMA-IR, TG, LDL-C, and SUA, a negative linear relativity of concentration of 25(OH)D3 with WHR, DBP, Fins, and TC (P 〈 0.05)was found, and a positive linear relativity between the concentration of 25(OH)D3 and HDL-C (P〈0.05). The MS group was subdivided into overweight and obese subgroups, and the concentration of 25 (OH)D3 of the two subgroups were not significantly different. Conclusion Vitamin D deficiency is common in the healthy elderly people. The concentration of vitamin D in the elderly patients with MS is significantly lower than in the healthy subjects. The concentration of 25 (OH)D3 is negatively associated with all components of MS, indicating that the 25(OH)Da deficiency is possibly a risk factor for MS.
